**Role Includes**:

- Provide direct patient care as outlined in the patient's care plan including but not limited to administration of medication, completion of treatments, and patient teaching related to care needs.
- Monitor, assess, and document changes in the patient's condition
- Assist with activities of daily living
- Collaborate with the inter-professional healthcare team to determine and implement the patient plan of care
- Additional opportunities as assigned by Nursing Supervisor
- Practice within the scope of practice set out by the College of Nurses of Ontario (CNO)

**Requirements**:

- Graduate of an accredited nursing program (Practical Nursing Diploma, Bachelors of Science in Nursing)
- Current registration with the College of Nurses, in good standing
- Participation in on-site training to support patient care needs with equipment including g-tube feeds, tracheostomy/oral suction machine, ventilator monitoring
- Valid work/study permit (if applicable)
- Current CPR/First Aid Certification
- Vulnerable Sector Screening (within 3 months of hire)
- Up-to-date Immunization Records (MMR, TDAP, Varicella, Hep B, Covid-19)
- Strong documentation skills
- Strong communication and problem solving skills
- Ability to follow client specific care plan
- Reliable transportation
