Casual Continuing Care Assistant- All Shifts

Main Activities

There are two homes
- one which provides transition, respite and palliative care. The other which provides a program to at risk moms who are prenatal and postnatal preparing them for transitioning to a new home with their infants.
- Provide supportive care to people living with or at high risk for HIV/AIDS who may also be affected by mental illness, addictions, poverty, homeless and other psycho social factors
- Provide supportive care through a patient centered model aimed at creating independence and autonomy while fostering an environment of acceptance and understanding
- Assist with and support clients with medication adherence
- Work with patients on individual goals and care plans aimed at recovery and management of their illness(s)
- Provide therapeutic counselling/assistance to clients who may present with multiple issues affecting their ability to cope and manage in the community
- Advocate on behalf of clients for additional service and resources, as required
- Personal care to patients including, assisting with grooming, bathing, personal hygiene, toileting, wound care, nutrition and all other activities associated with daily living
- Establish and maintain current, accurate, confidential files for each client
- Educate clients, families and support systems about Sanctum and what can be provided and when to access other resources
- Assist with the transition plans from Sanctum into the community set by the client and care team
- Follow medical instructions carefully and consistently
- Assist with meal preparation and planning, as well as other house hold duties such as cleaning and maintenance
- Organize time and resources based on the individual needs of clients
- Manage any unanticipated events or unstable situations

Therapy dogs on site at both homes

**Job Type**: Casual

**Salary**: $18.00-$23.00 per hour

- 8 hour shift
- Day shift
- Evening shift
- Night shift
- Weekends as needed

Supplemental pay types:
