**Summary**:
Back to Basic worker provides knowledgeable support to assist children and families to navigate the full range of federal and provincial health, social, and education services and supports provide through Jordan’s Principle to address children's needs.

**Requirements**:

- Completed Post-secondary degree in the Social Services with 1year direct social service experience; Combination of education and experience may be considered.
- Valid class 5 driver’s license and fully insured vehicle with $2,000,000.00 liability insurance.

**Key Responsibilities (but not limited to)**:

- Providing one on one case management support
- Working with children, families, and educators to identify the resources needed to address gaps and ensure positive educational outcomes.
- Providing navigation support to appropriate programs, departments and/or agencies and working with professional support in a responsive and coordinated manner.
- Coordinates and shares Jordan’s Principal program information,
- Builds and nurtures relationships across community-based programs and services, service providers, and First Nations, federal, provincial, and territorial programs, and services.
- Ensures children and families receive high quality culturally appropriate services and supports,
- Assists in identifying service providers to support children’s access to quality and culturally appropriate health, education, and social services, and supports across all stages and levels of care.
- Other duties as required or assigned.

**Salary**: $53,713.00-$69,146.00 per year
