Job Description
The Rehabilitation Assistant, working as an independent contractor, is an integral member of the Sinai Health to Home (SHtoH) point of care team at Circle of Care. With a focus on the social determinants of health, each member of the team works within the program guidelines to support patients recently discharged from Sinai Health. The Rehabilitation Assistant provides implements a treatment plan (either functional improvement or cognitive skill) developed by the OT/PT to specific patients. Working in close partnership with the OT and PT, the Rehabilitation Assistant contributes to successful outcomes while ensuring patient safety and effective reporting to mitigate risk. The program is intended to provide wrap-around services to support the transition from hospital to home and optimize functional recovery through self-efficacy and chronic disease management. In order to touch as many lives as possible and positively impact patient and family outcomes while on the program, team members will practice patient centred care to enable successful transitions into and out of the program.
The Rehabilitation Assistant may work as either an OTA or PTA depending on whether the delegation is provided by the OT or PT. OTs can delegate a home exercise program to improve overall deconditioning including stair safety, transfers with a gait aid, supervised walking, effective use of equipment to prevent falls, and ADL practice to increase functional independence. In addition, home safety activities for patients with cognitive impairment might be delegated as well. The PT will develop a focused treatment plan to restore functioning to arms and legs after fractures, improve balance, strength and walking tolerance, or energy conservation with patients with COPD or CHF. An exercise program may also involve stair safety, transfers with a gait aid or supervised walking to increase activity tolerance.

**Salary**: Commensurate with experience
**Hours of Work**: 15 visits/week with the potential for more. This role is community based and requires access to a vehicle to travel in Toronto.
**Reports to**: Manager, Client Services

**Responsibilities**:
Clinical Practice - Excellence and Quality Improvement
- In person home visits in assigned geographic area in the Toronto region.
- Follow the treatment program as outlined by the OT or PT and identify specific concerns or learning required before proceeding.
- Participate in joint visits for review of the treatment plan and attend monitoring sessions by phone or virtual sessions.
- Develop positive and respectful relationships with patients and families.
- Assess patients daily for any deterioration or possible risk and report to the Program Supervisor or assigned OT/PT.
- Use equipment in the home appropriately as directed and assess for safe use with every patient encounter.
- Use therapeutic communication with patients and their caregivers.

Clinical Excellence and Quality Improvement
- Review OTA/PTA Assignment of Care document and sign acceptance of treatment plan.
- Document OTA/PTA Treatment Plan after each patient visit. Submit to Circle of Care.
- Submit visit schedule and billings for payment.
- Adhere to verbal reporting requirements with respect to NSNF, Missed Care, Service Refusals or inability to contact patients, adverse events (including witnessed falls), complaints and compliments.

Information Management and Reporting
- Follow the organization’s policy on privacy and confidentiality and cybersecurity when using own devices for managing PHI.

Integrated Team and Clinical Leadership
- Strive to promote the values of the Sinai Health to Home program to promote an optimal patient experience, and continuity of care and information.
- Collaborate with OT/PT, communicate any risk situations with the Program Supervisor, and participate in huddles for patient updates.

Risk, Health, and Safety Management
- Identifying and reporting health and safety incidents and concerns in a timely manner to the appropriate supervisors and/or funders, documenting incidents in EasyCare and escalating appropriately to the designated supervisors as outlined in the Client Safety Reporting policy (C.01.38).
- Participating in health and safety processes and procedures
- Participating in maintaining a safe workplace environment by cultivating a positive safety culture and encouraging best practices to promote both staff and client safety and well-being
- Participating in all health and safety training initiatives on a regular basis.
- Taking proactive action against client incidents within your scope of practice.
- Developing a plan to identify, manage and/or minimize client safety risks or situations in adherence with risk management operations policies.
