**Job Summary**

The Social Worker is part of an interdisciplinary team providing support and coordination of discharge plans to the patients and their families receiving service from the Maternal Child Program.

**Qualifications**
- Masters of Social Work from an accredited university
- Membership in the Ontario Association of Social Workers
- Previous experience working with target population: infants and children up to the age of eighteen, women of childbearing age experiencing emotional and situational challenges related to their role as a parent.
- Experience working with child welfare agencies such as CAS.
- Experience supporting patients with substance abuse, domestic violence, psychiatric illnesses, postpartum depression and psychosis.
- Experience with palliative and bereavement support in the loss of an infant or child.
- Knowledge of adoption and surrogacy is an asset
- Demonstrated excellence in communication, both written and verbal
- Demonstrated problem assessment skills
- Demonstrated commitment to customer service and teamwork
- Demonstrated individual leadership skills

**Primary Responsibilities**
- Provides comprehensive, coordinated social work and discharge planning services to patients and their families in the Maternal child Program.
- Provides comprehensive psycho-social assessments of patients referred to the Maternal Child Social worker.
- Provides counseling services to patients and their families to assist them in adjusting to the effects of or complications related to childhood illness and childbearing/parenting. This would include ongoing emotional support and counseling for individual, marital and family conflict.
- Provides crisis and grief counseling for patient/family and team as required.
- Collaborates with hospital team and community support agencies such CAS and Public Health to assist patients with their discharge plans.
