**Job Description**:
**POSITION PURPOSE**:
Reporting to either a Supervisor, Property Manager, or Manager, is responsible for participating in the assessment and resolution of a diversity of complex issues in programs delivered by the Community & Health Services Department; developing and maintaining positive and cooperative partnerships in the Department and between local community service providers and clients; and assisting individuals and their families to identify alternative service options and to obtain the most appropriate services to enable them to maintain their housing, employment and/or child care services.

**MAJOR RESPONSIBILITIES**
- Facilitates/mediates the resolution of difficult, complex problems the client is facing which are resulting in barriers to service access, maintenance and/or self sufficiency (e.g. anti social, medical/psychological problems, tenancy, community problems, etc.) by working with individuals and families; investigating and determining causes, outlining implications regarding their future status and developing options/recommendations to de-escalate crisis situations and resolve issues.
- Provides instrumental and supportive counselling and case management to assist individuals and their families to function in the community.
- Provides crisis intervention in urgent client situations; investigates and recommends solutions, referrals etc. as required.
- Maintains records and statistical data; prepares reports as directed.
- Develops partnerships and maintains ongoing liaison with Regional delivered services, community and social agencies by facilitating and/or attending multi-agency (e.g. health, social, community, recreation, local law enforcement) meetings; supports and encourages Departmental staff to develop and maintain awareness of community issues and programs.
- Facilitates and supports the establishment of tenant organizations and/or recreation committees and acts as a resource person to the residents in assisting them to obtain funding for programs, integrate into the community, establish social clubs, etc. as assigned.
- Initiates/chairs or participates in case conferences; ensures that documentation and plan of action is completed; refers clients to other services where appropriate and maintains follow-up records.
- Performs other duties as assigned, in accordance with Branch and Department objectives.

**QUALIFICATIONS**
- Successful completion of a University Degree in Social Work from an accredited university.
- Must hold a Social Worker Certificate of Registration with the College of Certified Social Workers & Social Services Workers.
- Minimum two (2) years experience in the social services and community development field complemented by demonstrated skills in identifying and resolving a diversity of social issues related to community services and social housing.
- Satisfactory Police Criminal Background Check.
- Valid Ontario Class “G” driver’s licence and reliable vehicle for use on corporate business.
- Thorough knowledge of social service and community development principles and practices.
- In depth knowledge of community and social agencies covering a wide range of programs and issues such as life skills, resources for the frail and elderly, rehabilitation and counselling, childcare, recreation, mental health and development disabilities.
- Knowledge and demonstrated ability in corporate core competencies including customer focus, communication, collaboration and personal ownership.
