Due to our fast-growing patient care needs, CareKW is looking for a Clinical Nursing Supervisor to oversee CareKW's 25 bed Transitional Care Unit located in Kitchener, ON. We are looking for a compassionate person-centered nurse who is seeking an opportunity to use and enhance their knowledge, critical thinking skills, and leadership abilities to support our residents' care needs.

A transitional care unit is a short-term care facility designed to medically support patients transitioning from the hospital/home environment to long-term care. The CareKW team works to support the patient's continuity of care during their stay in the TCU as their healthcare needs change.

**Responsibilities**:

- Ensure the delivery of safe and competent patient care
- Review nursing referrals to determine patient suitability
- Communicate with Waterloo Wellington HCCS care coordinators and members of the inter professional team regarding patient care needs
- Develop, adjust, and maintain patient-centered care plans
- Supervise and support the unit nurse and care team to enhance overall patient wellbeing
- Participate in quality improvement initiatives
- Maintain flexibility to support with open shifts and scheduling as required to ensure patient safety
- Demonstrate competence within the scope of nursing practice
- Promote a supportive environment for both staff, patients, and families

**Qualifications**:

- Current Registration with the College of Nurses of Ontario
- Experience in complex continuing care, rehabilitation or medicine is considered an asset
- Experience in a leadership role is considered an asset
- Nursing knowledge and experience working with primarily a geriatric population
- Basic computer knowledge
- Current BCLS
- Ability to work independently in a multidisciplinary setting
- Positive attendance record
- Effective communication and documentation skills
- Ability to provide patient centered care
