**The Dufferin Area Family Health Team** currently has an opportunity for a **Mental Health Community Case Manager **(37.5 hours per week to join our compassionate, dynamic health care team dedicated to supporting patients for improved health and well being. The position will be located within Dufferin/Caledon.

The **Mental Health Community Case Manager **(1.0 FTE) will:

- Engage with and advocate for individualsexperiencing psychosocial stressors impacting their mental health including disability, illness, housing issues, unemployment, poverty, and/or loneliness
- Provide collaborative case management to assess needs, navigate services, and support individuals
- Partner with individuals to identify strengths and resources, develop goals, objectives, and take concrete action steps to address needs
- Demonstrate sensitivity to diversity, whether related to race, religion, culture, sexual orientation, age, and/or gender identity
- Collaborate with physicians, psychiatrists, and other multi-disciplinary team members

**EDUCATION/EXPERIENCE - **_**Required**_
- Bachelor of Social Work (BSW) required or Master of Social Work (MSW) preferred, with a minimum of 3+ years case management experience in supporting adults with mental health, addiction, developmental, or complex health needs
- Membership in good standing with OCSWSSW
- Current up-to-date knowledge of mental health and community services within Dufferin-Caledon
- Demonstrated ability to work independently, take initiative, be flexible, and collaborate well with others
- Experience in risk assessment and crisis intervention
- Ontario Driver’s License, valid insurance, and access to a reliable vehicle
- Proof of Covid 19 immunization and Criminal Records Check/Vulnerable Sector Screening
