The CMHC will also offer brief action planning and brokerage case management from a recovery perspective as a part of a collaborative and interdisciplinary circle of care. **D**uties and Responsibilities** - Act as a lead point of contact for access to SHIP Services in Dufferin - Respond to inquiries, "walk-in's," and calls regarding services and housing - Provide brief mental health counselling and short-term case management - Triage referrals, service requests and inquiries in a timely manner - Complete comprehensive assessments to support referrals to appropriate internal and external programs (i.e. supportive housing as well as clinical mental health services) - Provide consultation in collaboration with community partners to connect individuals to the services that best fit their needs - Conduct intake clinics at community service locations in collaboration with SHIP Central Intake and community partners **Qualifications** - Post-secondary education in Social Work, Health Services, Psychology or equivalent - 3+ years providing Mental Health assessments and/or case management - Professional designation preferred (i.e. Social Worker or Social Services Worker) - Training in assessment tools is an asset (LOCUS, OCAN, VI-SPDAT, HCR-20, and Suicide Risk Assessment) - Skilled in mental health counselling, support coordination, and supportive housing assessments - Knowledge in community health, healthcare, supportive housing and community resources - A vehicle, valid driver’s license, and adequate insurance coverage is required **Working Conditions and Physical Requirements** - Day shifts (Monday - Friday) with the ability to work excess/overtime hours where required - Travel by vehicle to community sites for department/program support as required **COVID-19 considerations**: - As part of SHIP’s commitment to health and safety, it is a condition of employment that you be fully vaccinated (2 doses, including 14 days since your second dose) against COVID-19 or have an exemption pursuant to the Ontario Human Rights Code. **Why Work For Us?** **Our Vision** Quality Housing, Quality Services, Quality Lives **Our Mission** To increase quality of life through health services and housing supports that promote mental and physical health and wellness **Our Culture** Leader and innovative partner in the community.
