Reporting to the Manager - Chronic Pain Management Program & Project ECHO, the Access Point Facilitator 2 will be responsible for processing referrals through Access Point Northwest, conducting decision guide intakes, and working closely with the Thunder Bay Regional Health Sciences Centre to refine the referral process between both programs.
**Qualifications**:
**Education/Experience**:
- Minimum qualification includes undergraduate degree and registered member in good standing with applicable professional college (e.g. College of Nurses of Ontario, Ontario College of Social Workers and Social Service Workers, College of Occupational Therapists of Ontario, College of Physiotherapists of Ontario)
- Knowledge and experience with referral processes, intake, assessment and triage
- Experience assessing clients with chronic pain as well as mild to moderate mental health issues
- Experience and understanding of the treatment of chronic pain and associated comorbidities
**Skills/Abilities**:
- Maintains an awareness of ethical guidelines and practices for the provision of effective client centered care
- Knowledge of relevant legislation, e.g., Mental Health Act, Personal Health Information Protection Act, Regulated Health Professions Act, Health Care Consent Act, and professional standards to ensure compliance and consistency in provision of client care
- System navigation and knowledge of community resources
- Passionate about contributing to positive social change and commitment to anti-oppression and anti-racism
- Knowledge of a range of theoretical perspectives, psychosocial rehabilitation principles, chronic disease self-management, holistic wellness approaches, and an ability to work in an inter-professional team environment
- Demonstrated excellent interpersonal communication skills (oral and written) to effectively communicate with clients, referents, consultants, partners, program managers, and directors
- Demonstrated experience with technology, documentation in the Electronic Medical Record and working with electronic databases
- Time management and organizational skills in order to effectively address and prioritize multiple tasks and timelines in a fast-paced environment
- Excellent oral and written communication and interpersonal skills, judgment, critical thinking skills, problem-solving and time management skills
- Ability to work closely with community partners and Access Point Facilitators
**Assets**:
- Experience working with Indigenous people and other diverse ethno racial and cultural backgrounds is considered an asset
- Experience working with 2LGBTQ+ individuals is considered an asset
- Experience working with clients living with chronic pain
**CONDITION OF EMPLOYMENT**
- This position shall be conditional upon a satisfactory Police Vulnerable Sector Check (PVSC) where indicated, to ensure the absence of relevant criminal convictions.
- May be required to travel to various work locations and provide own transportation
