We currently have an exciting opportunity for a **Full-Time** **Leader, Local Specialized Community Services Programs (SCSP) in the Tri-Cities.**
This role provides leadership to the New West Diabetes Clinic, Tri Cities Diabetes Clinic and the Pulmonary Respiratory Clinic. This role also leads and plans the development of an integrated health system focused on the provision of care for complex medically frail seniors.

The Leader, Local Specialized Community Services Programs (SCSP) provides leadership and oversight to the development, implementation and operationalization of Specialized Community Services Programs within the geographical area and in collaboration with Fraser Health SCSP network and clinical leaders The Leader collaborates and builds strong partnerships with local operational leaders project leads, corporate and support departments, physicians, Division of Family Practice partners, and community partners, always remaining aware of and sensitive to the interests of the various partners. Develops strategies aimed at achieving meaningful health outcomes and quality services that are timely and consistent with Ministry of Health objectives. Oversees communications and knowledge transfer required to ensure effective services across the continuum of care. The Leader is also responsible for providing leadership and management to assigned community clinical teams, as well as overseeing the associated staff and budget.
**Responsibilities**:
- Leads the planning, development, implementation and operationalization of Specialized Community Services Programs SCSP within the geographical area, in collaboration with Fraser Health SCSP network, ensuring models of care delivery are meeting the local needs of adults with complex medical conditions and/or frailty and are consistent with Ministry of Health and Fraser Health guidelines.
- Provides direct leadership and management of assigned clinical operation(s). Manages resources and oversees the assigned staff/budget, including operational and administrative activities. Acts as a resource to care teams, provides expert consultative advice to other health care practitioners and acts as a resource for the assigned clinical operation(s) or service contracts. Collaborates and builds strong partnerships to ensure collaboration in the provision of seamless, quality patient transitions. The various partnerships include internal stakeholders such as Long Term Care, Assisted Living, Hospice, Home Health/Support, Respite, as well as external partners such as Primary Care Networks, First Nations, Division of Family Practice.
- Develops, implements and manages a project management framework with templates, samples and guidance including methodology mentorship, in alignment with Fraser Health project management philosophies, that will guide the development and implementation of SCSPs.
- Develops strategies aimed at achieving meaningful health outcomes and quality services that link to key service attributes including service accessibility, appropriateness, acceptability, safety, and efficiency.
