**Job Purpose**

We are looking for a passionate and empathetic Support Worker to join our team, a non-profit organization dedicated to supporting individuals facing various barriers to achieve their goals and lead fulfilling lives. As a Support Worker, you will assist and empower our clients to overcome challenges and build resilience. Your primary objective will be to provide personalized support and guidance to individuals, ensuring they access the necessary resources, services, and opportunities to break down barriers and thrive.

**Duties and Responsibilities**

1. Client Support:

- Build strong rapport and establish trusting relationships with clients, understanding their unique barriers and challenges.
- Conduct comprehensive assessments to identify individual needs, goals, and strengths.
- Develop personalized support plans collaboratively with clients, setting achievable objectives.
- Assist clients in accessing resources, services, and programs aligned with their goals.
- Provide emotional support and reassurance, actively listening and responding to clients' concerns.
- Encourage and empower clients to develop self-advocacy skills and strategies to overcome barriers.

2. Advocacy and Liaison:

- Collaborate with community agencies and service providers to coordinate support for clients.
- Collaborate with the alliance and facilitate communication and cooperation between clients and various community stakeholders to address barriers effectively.
- Attend meetings, workshops, and community events to represent Breaking Down Barriers and advocate for clients.

3. Training and Education:

- Deliver educational workshops and training sessions to clients on various topics, such as self-advocacy, life skills, and community integration.
- Develop and update educational materials and resources to address clients' specific needs.
- Provide guidance on accessing educational opportunities, vocational training, and career development programs.

4. Documentation and Reporting:

- Maintain accurate and up-to-date case notes and records of client interactions and progress.
- Prepare reports on individual outcomes and program effectiveness, highlighting successes, challenges, and recommendations.
- Ensure compliance with all relevant policies, procedures, and regulations related to client data protection and confidentiality.

**Qualifications**
- A diploma in social work, psychology, counseling, or a related field is preferred.
- Proven experience working in a support worker or similar role, preferably with individuals facing multiple barriers or disabilities.
- Knowledge of community resources, support services, and benefits available to individuals with diverse needs.
- Excellent interpersonal and communication skills, with the ability to build relationships and establish rapport with diverse clientele.
- Demonstrated empathy, patience, and a non-judgmental approach towards supporting individuals overcoming barriers.
- Strong problem-solving skills, with the ability to think creatively and adapt to changing situations.
- Ability to work independently and as part of a team, managing multiple priorities.
- Valid driver's license and access to a reliable vehicle.

**Working Conditions & Physical Requirements**

The Support Worker will work day shifts with occasional evenings if needed for programming. Working mostly at a desk with occasional lifting of donation bags when required. There is travel to Northern Ontario Communities when needed to meet with partnering organizations and foster relationships with community partners.

**Job Types**: Full-time, Fixed term contract
Contract length: 30 months

Pay: From $23.33 per hour

Expected hours: 32 per week
