**About the Opportunity**:
The philosophy of the Case Worker is grounded in person centered care and Home First by maximizing available services and resources to support clients and families in the community.
Reporting to the Manager, Continuing Care, the Case Worker is an unregulated member of the case management team. The Case Worker works collaboratively within an interprofessional case management team to provide an array of services and programs offered by NSH Continuing Care and Department of Seniors & Long-term Care.
Duties of the Case Worker include but are not limited to:
- Liaises with client, family, caregiver on an ongoing basis and is responsible to manage a caseload of clients that are deemed to have stable and predictable service needs
- Using approved and standardized tools, collects information from client, family, caregiver and other care providers to support a needs assessment for service provision
- Facilitates access to services by making referrals to and collaborating with contracted service providers and voluntary-based resources
- Evaluates program and service needs according to established standards, including when the client’s needs or situation have become less stable and predictable.
- Maintains accurate written / verbal communication with clients, family, caregivers, care providers and other team members
**About You**:
We would love to hear from you if you have the following:
- Diploma in Social Services, Disability Management Program, or related program
- Minimum three (3) years’ related job experience
- CPR and Emergency First Aid
- Education and/or experience in dementia care preferred
- Experience working in partnership with individuals to identify needs, develop support plans, and maintain supportive, respectful and professional working relationships
- Understanding of family/relationship dynamics
- Understanding of societal/structural impacts (e.g., culture, ethnicity, education) and demonstrated ability to engage individuals using a person-centered approach
- Demonstrated knowledge and/or experience in a community or long-term care setting serving vulnerable populations; or knowledge and/or experience in case management (intake, assessment, service planning, implementation, monitoring and ongoing case management, discharge).
- Intermediate-level computer knowledge and use of Windows, MS Edge, Outlook, other technology
- Knowledge of community agencies and resources
- Knowledge of trauma-informed care principles and practices
- Excellent verbal and written communication skills
- Must be a team player who applies a collaborative approach to their work, displays sound judgment, critical thinking, creativity, problem solving and effective time management
- Demonstrated ability to handle stressful situations, workloads and adapt to changing environments and program demands
- Valid driver's license and access to a reliable vehicle, travel is required
- Competencies in other languages an asset, French preferred

**Hours**:
- Full Time, Long Assignment; 70 hours bi-weekly
- Approximate two year assignment
- Hours of work may include days, evenings, weekends, and holidays
**Compensation and Incentives**:
$28.68 - $32.49 hourly
