Lynwood Charlton Centre (LCC) is a publicly funded charitable organization in the City of Hamilton which provides a spectrum of innovative and evidence-based, mental health services, including residential, day treatment, community-based services including in-home and school-based services, to children, youth, families and the community.
The Centre is funded and licensed by the Ministry for Children and Youth Services and accredited by the Canadian Centre for Accreditation. We are currently looking for **Full Time Permanent Family Resource Clinician** to join our GPS Complex Needs program in Hamilton.
**Role Overview**
Working under the direction of the Director of Program Operations, the Family Resource Clinician is responsible for case management, service coordination and navigation, through collaborative community processes and to participate in program development and evaluation. In addition, to provide coaching to sustain and enhance strategies and skills developed by the family through previous involvement with intensive service(s).

**Main Duties and Responsibilities**
- Contribute to the overall case planning as generated by the child/youth, family and the interdisciplinary team
- Utilize referral information and explore the child/youth and families’ previous involvement in community services to inform a plan that sustains and enhances the family’s skills and strategies in such a way that is meaningful and relevant to the family
- Utilization of assessments to provide recommendations and education to Children, youth, families staff and community collaterals on treatment modalities.
- Provide resources, and treatments utilizing a wide range of treatment modalities appropriate to the needs and strengths of the child/youth and family.
- Support and provide advocacy for the child/youth and family in assessing eligibility and referral to relevant community services and resources to address child/youth and family’s needs and goals
- Provide respite coordination, where appropriate, including accessing and tracking relevant funding and supporting the family to recruit respite workers
- Employ a systemic, family centered approach to develop a strengths-based plan which draws on already established treatment interventions appropriate to the needs and strengths of the child/youth and family
- Provide direct support and coaching/guidance to families for the sustainability of behavioral and treatment approaches and interventions to the child/youth in their home, school and community
- Provision of information and education to children/youth, families and collaterals to support the implementation of the plan
- Maintain clinical records, assessments and reports according to agency and program policy and procedures.
- Other duties as assigned
**Experience/Skills Required**
- MSW/BSW or equivalent with several years of related clinical experience
- Registered and in good standing with: CCPA, Ontario College of SWSSW. CRPO or equivalent
- Certification in CHYMH or certification obtained within 3 months of hire
- Certification in SMG Crisis Intervention Program
- Experience working with families and children and/or youth in a treatment/counselling setting.
- Knowledge of child and youth mental health and relevant evidence informed treatment interventions
- Experience working with children/youth in family centered approach
