**Summary**:
Native Child and Family Services of Toronto strives to provide a life of quality, well-being, caring and healing for our children and families in the Toronto Indigenous Community. We do this by creating a service model that is culture-based and respects the values of Native people, the extended family, and the right to self-determination and through the integration of holistic, culturally based prevention services that are child centred, family focused, and community driven.

**Primary Responsibilities**:
Under the direction of the Supervisor, Youth Engagement, the Youth in Transition Worker will:

- Work with Indigenous youth between the ages of 16 and 24 who are leaving the care of Child Welfare and are transitioning to independent adulthood.
- Support eligible youth leaving care to identify, access and navigate adult service systems relevant to their needs and transition to adulthood.
- Act as a source of integrated information, referrals and short-term case management that brings Indigenous youth and services together to meet vital needs.
- Connect youth to existing supports and resources in the community, such as housing, education, employment services and training, life skills training, health and mental health services and legal services.
- Collect and maintain data on program activities, expenditures and outcomes, and prepare quarterly reports for submission to the ministry using a reporting template provided by the ministry.
- Record statistics in relation to program participants.
- Maintain a collective working relationship with other professionals both within and outside of NCFST.
- Attend partnership and other related meetings, as required.
- Other duties as assigned.

**QUALIFICATIONS, KNOWLEDGE, AND SKILLS**:

- A Child and Youth Worker Diploma and 2 years of related experience.
- Alternatively, an equivalent combination of relevant education in a related field and experience working with Indigenous youth may be considered.
- Current Vulnerable Sector Police Record Check or a willingness to obtain one.
- Knowledge of Indigenous and mainstream community resources.
- Knowledge of Indigenous cultures and understanding of historic context.
- Knowledge of and sensitivity to the life experiences and needs of the youth community, particularly crown wards and youth leaving the care of a CAS.
- Knowledge of social risk factors in relation to high-priority neighbourhoods.
- Knowledge and skills in the delivery of group programs and workshops that incorporate strength based and evidence based practices.
- Understanding and/or experience in working with the two-spirit (LGQBT) Lesbian, Gay, Queer, Bi-Sexual and Transgendered community.
- Excellent organizational and oral/verbal/written communication skills.
- Excellent computer and technical skills.
- Initiative and respect for confidentiality.
- Experience in the Stages of Change Model of outreach practices is an asset.
- Availability to work evenings and weekends.
- Flexibility and ability to work within a team approach of service delivery.
- Demonstrated understanding, and commitment to, integrating the Native Child and Family Services of Toronto Mission and values in to practice, service and relationships.
- Demonstrated understanding of workplace Health and Safety practices and understanding of an employee’s responsibility under current legislation.
- Ability to work effectively with all levels of staff, to maintain effective communication and working relationships, demonstrating strong interpersonal skills, tact, sensitivity and build strong internal relationships.
