**About the Role**:Community Support Worker (CSW)

This position is focused on supporting youth and young adults who are in conflict with the law to reintegrate into the community with success, purpose and meaning. Working collaboratively with
Probation Officers and other key partners, the CSW delivers transition services to young persons throughout the Greater Toronto Area, through direct service, coordination, advocacy and consultation.
This position focuses on securing continuity of education, housing, employment, health and other critical services for youth and young adults in a multidisciplinary, team-based approach.

The CSW manages a caseload of young persons and addresses their needs by engaging with them to develop meaningful, goal-driven, tailored service plans, to meet each young person’s unique needs and strengths, with the goal of successful reintegration into the community. The process is supported through screening, assessment, services, referrals and program delivery.

In addition, the CSW will have a specific focus on recreational support. They will work with youth/young adults to become engaged in ongoing recreational activities establishing positive community linkages and encouraging positive behaviour. The CSW’s expert input around recreational activities supports mental health, self-care, and mindfulness to address the needs and strengths of the young person.

**About You**:Knowledge, Skills and Experience

1. Demonstrated experience with case management, mentorship and counselling with at-risk youth and/or young adults

2. Demonstrated experience with and sensitivity to the client population’s cultural and socio
- economic characteristics

3. Demonstrated ability to work independently, while engaging with partners, colleagues and supervisors as needed, to maximize Case Management effectiveness.

4. College diploma or University degree in a related field
5. Minimum of 1-3 years’ experience working directly with young persons in conflict with the law.
6. Excellent administrative, organizational, communication and leadership skills
7. Proficient knowledge and use of available computer technology and systems
8. Knowledge of relevant legislation and guidelines, including but not limited to: Youth Criminal

Justice Act, Child & Family Services Act, Ministry of Correctional Services Act, Education Act,
Ontario including section 23 programs

9. Valid Driver’s License, clean driving record and access to a personal vehicle (with mileage reimbursement)

10. Police Record Check with vulnerable sector screening
11. Able to work 35 hours a week with flexibility to work evenings or weekends as required
12. Minimum of two COVID-19 vaccinations

**About Salary and Benefits**:

- $48,000 to $52,000 annually
