Responsibilities include working collaboratively with numerous health care agencies such as the Local Health Integration Network (LHIN), Health Link, Ontario Health, family health care teams, etc., to co-ordinate care and other services for patients living with complex underlying conditions. Some of the accountabilities for this role are: - Supervises and leads the CPP in accordance with current Community Paramedicine standards, regulations and Corporate policies. - Supervises assigned staff including staff development, performance management/review, coaching/mentoring, health & safety, collective agreement administration, discipline, and termination decisions. - Supports existing health services and provides integrated health services in partnership with other health professionals and organizations. - Extends access to health services delivery in high risk populations, including primary care, public health, disease management, prevention, wellness, and mental health. - Ensures that CPPs align with the Community Paramedicine framework as outlined by the Ministry of Health and Long Term Care (MOHLTC) and the Northwest LHIN. - Develops and implements new initiatives, pilots and programs to assist in improving the health status of targeted residents as approved by Senior Management. - Oversees data collection and analysis pertaining to community paramedicine activities, for program evaluation and quality assurance. - Provides detailed qualitative and quantitative reporting on community paramedicine activities to City Council, LHIN, MOHLTC, etc. - Engages the community through initiatives, programs and educational presentations, including diabetes prevention/treatment, hypertension, falls assessment, injury evaluation, geriatric frailty visits, and nutrition. - Must be qualified as an A-EMCA and have a minimum of five (5) years’ experience as a paramedic - Knowledge of land ambulance legislation, regulations, and current practices pertaining to land ambulance services and personnel - Excellent interpersonal skills and the ability to relate to the public, staff and associated health care providers - Must possess superior analytical, multi-tasking, organization, customer service, and teamwork skills - Must possess and maintain a valid Ontario Class “F” driver's licence or equivalent For a detailed job description, click here. **Why SNEMS?** With over 200 front line paramedics, who are highly skilled and trained, our service provides unwavering dedication, compassion and medical attention to a variety of injuries and conditions.
