**The successful applicant will display proficiency in the following accountabilities**:

- **Support**:_
- Ensure the assessment of the individual’s support needs, including their quality of life and independent living skills
- Ensure the ongoing development of the Support Plan and Support Agreement, as well as their effective implementation
- Along with the rest of the team, provide support to the individual in all aspects of community living as identified in their support agreement
- Ensure family contact is maintained
- Ensure medical needs are met
- Ensure that preparation is made for all aspects of life such as, funeral arrangements, wills, etc.
- Perform additional job duties as assigned by the Manager

**Empower**:

- Ensure the individual is assisted in setting and achieving their personal goals(i.e. work, recreation)
- Act as an advocate
- Teach self-advocacy skills
- Develop the individual’s network of personal advocates

**Community**:

- Ensure that the Team is supporting each person to develop meaningful relationships with friends, families, neighbors, and co-workers
- Ensure that the team is assisting each person to connect to community groups schools, agencies and employers for opportunities to engage in their community
- Ensure that each person supported is assisted in locating personal and social activities and relationships

**Organizational**:

- Encourage a culture of positivity and professionalism, demonstrated by personal example and the establishment of a positive leadership culture
- Encourage all employees through direct supervision to be an active participant in not only the work of the team, but the Mission, Vision and Values of ISSI
- Represent the support team and each person supported as part of the ISSI Leadership Team. As a member of the Leadership Team, support and champion the service and support philosophy of the organization.
- Represent the support team and each person supported by providing essential input into the annual ISSI Strategic Plan and Vision.
- Follow agency policy and procedure regarding recording and reporting
- Be aware of and adhere to ISSI policy and procedures
- Maintain confidentiality as according to ISSI policies and procedures
- Be familiar with related legislation and ministry guidelines
- Facilitate staff meetings
- Deal with minor staffing issues
- Keep team informed of information given from Manager and/or Director of Community Involvement & Support Services
- Develop and maintain team work schedules around the needs of the individual(s) supported, while following financial guidelines established in program budgets. Schedules will be reviewed and approved by Managers
- Assist in finding replacement staff when necessary
- Complete daily approvals in Inclusion System by the end of each pay period
- Complete monthly reconciliations of ledgers
- Oversee day-to-day operation of the team
- Inform Manager of household and daily concerns
- Attend relevant meetings as scheduled
- Attend staff development events as according to staff training plan
- Ensure all agency property is maintained to acceptable standards
- Participate as part of the On-Call Team
- Delegate tasks to other team members as necessary, encouraging collaboration and cooperation within the team

**The successful applicant will hold the following** **qualifications**:

- A degree or diploma in Human Services with two years experience in providing direct support in a community based agency, or a comparable combination of experience and education
- Current First Aid/CPR and Crisis Intervention (CPI) certificates
- Knowledge of Community Services
- Ability to act as an advocate for individuals and staff, facilitating individual growth and change
- Demonstrated willingness to pursue personal development and enhancement of skills
- Demonstrated ability to work as part of a team, encouraging unity and a positive, progressive atmosphere
- Proven leadership ability, showing initiative, strong interpersonal skills and creativity
- Valid -Full G Driver's License
- Vulnerable Sector Criminal Reference Check, or a willingness to obtain this
