**job summary**

Assist with the care of clients in accordance with the identified service plan, goals, and objectives, policies and procedures consistent with the program mandate and senior services.

Provide to clients living at Deer Park Suites; assisted living services in a manner that promotes wellness and maintains independence. Perform required services identified in individual service plans that fall under the Assisted Living program mandate encouraging client involvement and degrees of assistance in the areas of: homemaking assistance (laundry/housekeeping); activities of daily living (bathing, foot care, dressing, toileting, mobility & feeding support); medication prompts; attendant care; meal/nourishment preparation and dispensing; life enrichment activities to address psycho/social, health, wellness, and/or community engagement needs. Complete PSW-enhanced skill training on a client-specific basis: assistance with eye/ear drops, blood pressure check, and colostomy care. Work with community health partner providers to create an integrated service plan of care as identified.

Report and record observations of client social, physical, mental changes, and/or service changes as a result of client direction. Complete documentation as per policy/established practice guidelines.

**education**

• Grade 12 diploma or equivalent
• Completion of an approved Community College - Personal Support Worker (PSW) Certificate Course
• Additional courses in enhanced PSW / gerontology an asset

**knowledge/experience**

• Minimum of 3 months of current experience in a Supportive Housing/Assisted Living Program, Adult Day Program, or related experience in delivering personal support services to seniors in a personal home setting

• Must have an excellent rapport with people and demonstrated working ability to understand the unique needs of the elderly
• Must be willing to obtain the PSW enhanced skills training
• Demonstrated commitment to the VALUES of service, honesty, choice, partnership and respect for clients of Supportive Housing
• Demonstrated flexibility and resourcefulness in achieving client independence and quality of life outcomes

• Must be physically and mentally capable of performing the duties of the position.

Responsibilities

• Assist clients as scheduled with service needs as identified on the client service plan.
• Under the supervision of a community professional and as determined by the Manager or designate, perform procedures as directed and trained in response to the assessed need of an individual (i.e. blood pressure checks, glucose monitoring, surveillance swabs, pulse respiration, urine tests, eye/ear drops, weights).
• Assist with personal care to the level and ability of the client optimizing independent functioning and consistent with the authorized service plan including: tub bath/ bed bath/ shower, mouth care, un/dressing, grooming, foot/nail care transfer assistance, assistance with medication/medication prompts; skin care, continence care routines as identified. Be responsive to unscheduled service requests that arise.
• As an unregulated care provider responsible for all aspects of medication prompting as per practice guidelines for those clients identified for this service and pre-arranged on the established medication system (i.e. vendor).
• Monitor overall health and well-being of all clients, identify changes in presentation, service supports required, and report changes according to process guidelines.
