**Job Description**:
JOB SUMMARY
Assist with the care of clients in accordance with the identified service plan, goals, and objectives, policies and procedures consistent with the program mandate and senior services.
Provide to clients living at Deer Park Suites; assisted living services in a manner that promotes wellness and maintains independence. Perform required services identified in individual service plans that fall under the Assisted Living program mandate encouraging client involvement and degrees of assistance in the areas of: homemaking assistance (laundry/housekeeping); activities of daily living (bathing, foot care, dressing, toileting, mobility & feeding support); medication prompts; attendant care; meal/nourishment preparation and dispensing; life enrichment activities to address psycho/social, health, wellness, and/or community engagement needs. Complete PSW-enhanced skill training on a client-specific basis: assistance with eye/ear drops, blood pressure check, and colostomy care. Work with community health partner providers to create an integrated service plan of care as identified.
Report and record observations of client social, physical, mental changes, and/or service changes as a result of client direction. Complete documentation as per policy/established practice guidelines.
