Community Support Worker**
- Job Summary - Community Inclusion_

Plans, organizes, implements and evaluates community based recreational, social and/or educational activities. Encourages and facilitates individuals’ with developmental disabilities to participate in those activities.
- Job Summary - Residential_

Assists residents to live successfully in residential settings such as group homes. Ensures that residents’ physical, emotional, social, education, medical needs are met. Assists residents to enhance quality of life with activities of daily living and the development of life skills.

**Reports To**:Coordinator Adult Day Team, Residential Manager, or Program Director

**Key Duties and Responsibilities** **- day services and residential**
- Participate in the development of personal service plans, goal setting and program planning
- Assists individuals with activities of daily living such as feeding, lifts, transfers, hygiene, grooming and toileting. This may include delegated tasks such as gastrostomy feeding, tracheostomy management, and suction and bowel management.
- Plan, organize, implement, and evaluate vocational, recreational, and educational activities
- Administer medication in accordance with established policy
- Provide behavioural support for clients in accordance with behavioural support plans
- Identify services in the community that will meet clients’ needs
- Accompany and/or transport clients to activities or appointments
- Recognize, analyze and deal with potential emergency situations
- Provide feedback and support to clients and/or their families and other service providers
- Ensure health and safety standards are maintained
- Maintain related records and statistics and produce reports as required
- Liaison with families, other community service providers and professionals
- Ensure that CCS Mission Statement and all policies are maintained especially the client’s right to choice, confidentiality, and privacy
- Maintain high standards of teamwork
- Recommend changes as appropriate
- Carry out other duties as assigned

**Specific Duties and Responsibilities - day services**
- Provide on a daily basis one-to-one or group activities that support the development of life skills, vocational possibilities, recreation and socialization

**Specific Duties and Responsibilities - residential**
- Provides life skills training such as meal preparation, housekeeping, personal care skills and personal finance and implement personal service plans
- Performs residence maintenance and housekeeping duties such as laundry, sweeping, mopping floors, mowing lawn, snow removal, inventory, shopping, cleaning equipment and meal preparation

**Qualifications: Education, Training and Experience**
- Community Support Worker certificate or equivalent
- WCB approved First Aid and CPR certification
- Food Safe Certificate
- Class 4 driver’s license and own vehicle
- three month’s experience delivering similar services
- experience in a community inclusion or residential setting

- effective interpersonal communication skills
- demonstrated ability to work alone and unsupervised, as well as part of a team
- demonstrated good time and general management skills
- basic computer skills preferably experience with Microsoft Office
