**Job Summary**

**Responsibilities**
- Assist clients with Activities of Daily Living (ADLs), including bathing, dressing, grooming, and mobility support
- Provide meal preparation and nutritional support tailored to individual dietary needs
- Monitor vital signs and observe patient conditions to ensure safety and well-being
- Administer medications accurately following care plans and HIPAA regulations
- Utilize equipment such as Hoyer lifts or other assistive devices for safe patient transfers
- Maintain detailed documentation of patient care activities and progress in compliance with healthcare standards
- Support clients with cognitive impairments such as dementia or Alzheimer's through memory care techniques
- Assist with light housekeeping tasks including cleaning to maintain a safe and comfortable environment
- Collaborate with healthcare professionals to develop and implement personalized care plans
- Ensure compliance with all safety protocols, including CPR and first aid procedures
- Provide emotional support and companionship to promote mental health and social engagement

**Qualifications**
- Proven experience in home health, assisted living, nursing home, or long-term care environments
- Knowledge of Alzheimer’s care, dementia care, hospice care, autism support, or working with people with developmental disabilities is preferred
- Valid certification in CPR and First Aid required
- Experience with medication administration, vital signs monitoring, and patient observation essential
- Ability to lift heavy objects safely (heavy lifting) when necessary for patient transfers or mobility assistance
- Strong communication skills in Cantonese or Mandarin; proficiency in both languages is highly desirable
- Familiarity with HIPAA regulations and maintaining patient confidentiality
- Compassionate attitude with excellent caregiving skills and patience for working with seniors or individuals with disabilities
- Ability to work independently while following established care plans and protocols

This position is integral to providing compassionate, respectful, and professional support that enhances the quality of life for clients requiring home-based assistance.
