The Community Outreach Center Clinic was created to provide accessible primary care services to the most vulnerable in our community; the unhoused, housing insecure and transient populations. The Community Outreach Center (COC) is used by this community for daytime respite, communion, and connection to social services.

The Community Outreach Center Clinic provides urgent care and transitional primary care for clients of the Community Outreach Center. We are not a family practice; however, we provide care for unaffiliated patients while they wait to become affiliated with primary care providers. Furthermore, we coordinate care and co-manage affiliated clients who have difficulty accessing their family doctors.

Our care is colloquially known as ‘Street Medicine’, a practice that focuses on integrating primary care, urgent/emergent care, psychiatric and addictions services for vulnerable populations struggling with housing insecurity.

The MOA acts as a key administrative support to the Community Outreach Centre clinical team. The MOA works alongside doctors, nurse practitioners, registered nurses, registered social workers, psychiatrists, case managers, and other multi-disciplinary staff members.

The MOA is responsible for the day-to-day organization and administration of clinical services within a community based medical clinic. The MOA has an understanding and respect for mental illness, substance abuse disorders, and complex co-occurring health conditions. The MOA has a strong ability to work independently, to take initiative, and to work within a multi-disciplinary team.
- **Primary Responsibilities (Including but not limited to)**:_
- Creating weekly practitioner and nursing clinic schedules.
- Booking patient appointments and communicating with case management team.
- Scheduling monthly medical, psychiatry, social work, and addictions clinics.
- Updating patient charts and files.
- Maintaining patient follow ups schedules; patient medical histories; medication lists; and updating pertinent patient social determinants of health histories.
- Receiving/Organizing/Triaging clinic referral documents.
- **EMR physician inbox management**
- **Follow up of referrals, lab and imaging requests on the Electronic medical record on EMR platforms**
- **Communicating with pharmacist/ pharmacy re medication records and prescription follow up**
- Initial point of contact for patients, members of the public and other health care facilities.
- Initial contact for directing queries for all programing/policies within the clinic
- Monitor waiting areas to ensure patients are registered/checked in
- Communicate with staff and public in a respectful, confidential manner
- Maintain confidential work area
- Upkeep of their workspace ensuring adequate supplies and notifying admin assistants of supplies needed to be ordered
- Update and maintain wait lists and/or statistical data as requested/required
- Receive and sort incoming mail/supplies and distribute accordingly
- Scan/File documents received to the clinic.
- Ability and knowledge of services available to direct inquiries to appropriate team member.
- Register patients in electronic schedule system using the two patient identifier methods.
- Create medical chart, electronically/paper, based on new and follow up patients, also to include follow up on missing information, demographics and request pertinent info as required.
- Retrieval of historical patient info/chart for patient/staff as required via RIM process.
- Obtain patient consent for medical chart transfers/information.
- Relay instructions to patients for lab and diagnostic testing and necessary preparations
- Accurate and timely interpretation of physician’s orders for follow up
- Open and close health centre.
- Maintain communication with courier(s) for incoming/outgoing mail and supplies and follow up on delays.
- Report all equipment/telephone malfunctions to supervisor/ITSS as appropriate.
- Relieve/backfill other support staff in the clinic as required.
- Develop templates to print/send physician notes to their appropriate source.
- Care coordination including helping patients follow up on and travel to appointments.
- Taking part in clinic rounds, bimonthly team meeting and help coordinate educational rounds.
- **Education/Experience**:_
- Successful completion of a Medical Office Assistant training program (or equivalent).
- Must be legally able to work in Canada.
- Experience on multiple electronic platforms which may include DIS, CIS, CHR and ICore, is an asset
- Experience working in Primary Care, Mental Health and Addictions, Street Health/ Outreach Medicine is an asset
- Experience working in an office setting, preferably a medical office or clinic is an asset
- Mental Health and Addictions training is an asset
- **Other Qualifications**:_
- Training on Electronic medical record including CHR/Telus is an assest but training will also be provided
- Additional relevant education and experience will be
