**Position Summary**:
Reporting to the Manager of Health Information Management, The Health Records Technician is responsible for accurately coding and abstracting inpatient (DAD) and ambulatory (NACRS) health records according to the International Classification of Diseases and Health-Related Problems - 10th Revision, Canada (ICD-10-CA) and the Canadian Classification of Health Interventions (CCI) in accordance with Canadian Coding Standards and CIHI (Canadian Institute for Health Information) guidelines. This role ensures high-quality coded data that supports hospital funding, research, and quality improvement initiatives while maintaining compliance with provincial and national health data reporting requirements
**Responsibilities**:
- Applies appropriate codes to patient records according to ICD-10 and CCI specifications, Canadian Coding Standards, Ontario Ministry of Health and Long-Term Care mandates and SMGH specific requirements as required.
- Completes CIHI abstracts and corrections applicable to records coded, as required.
- Ensure proper sequencing of codes for optimal reimbursement and compliance
- Work closely with HIM Team to maintain coding consistency across settings
- Coordinate with health data analysts to improve coding accuracy and efficiency. Participate in department meetings to discuss coding trends, policy updates, and process improvements
- Stay updated on ICD-10-CA/CCI, CIHI Guidelines and Canadian Coding Standard Changes, Case Mix Methodologies - Maintain coding certifications ( e.g, CHIM, CCS) through continuing education
- Participate in professional organizations (e.g, CHIMA, CIHI Training Sessions)
- Other Duties as assigned
**Position Requirements**:
- Certified Health Information Management (CHIM) credential from the Canadian College of Health Information Management (CCHIM).
- Successful completion and Graduate of an accredited Health Information Management (HIM) program.
- Member in good standing with Canada’s Health Information Management Association (CHIMA).
- Minimum 1-3 years of coding experience in an acute care, outpatient, or rehabilitation setting.
- Extensive knowledge of ICD-10-CA and CCI coding classifications and Canadian Coding Standards.
- Experience coding using CIHI guidelines, Case Mix Group (CMG), Comprehensive Ambulatory Classification System (CACS), Health Indicator Group (HIG) Weights, and Resource Intensity Weights (RIW).
- Proficiency in Cerner PowerChart and 3M Codefinder for electronic health records (EHR) and computer-assisted coding (CAC).
- Strong skills in Microsoft Office (Excel, Word, Outlook, PowerPoint) for data reporting and analysis.
- Ability to work closely with physicians, CDI specialists, health data analysts, and hospital administration.
- Strong analytical and problem-solving skills to resolve coding discrepancies and documentation queries.Effective verbal and written communication skills for collaboration and provider education.
