The Indigenous Navigator will seek to integrate provincial, federal and other community health and social support services across the care continuum. This role will improve access to quality care through a variety of providers, both clinical and non-clinical addressing the physical, social, emotional and spiritual care needs of the client.
The Indigenous Navigator will perform resource support and advocacy for Indigenous patients, caregivers, and their families by appropriately responding to their health needs and goals. The Navigator will assist in bridging the service gaps between Indigenous communities and health care providers by facilitating linkages to services that may include (but are not limited to) mental health and addictions, chronic disease management, primary care and acute care. The overall goal of this position is to assist patients to move easily through the healthcare system in a culturally safe and relevant manner. This position will collaborate with multiple stakeholders, internal and external, to provide a broad range of support services for a number of initiatives relevant to improving health outcomes for First Nation, Inuit, and Metis (FNIM) communities.
Regular Full-Time
Rate of Pay: $44.33 per hour - $55.41 per hour
Hours of Work: 37.5 hours per week
**Qualifications**:
- Successful completion of Bachelor?s degree in Social Science or relevant healthcare field
- Current registration as a regulated health care professional in Ontario is preferred
- Palliative Care for Front Line Workers in First Nations Communities_ _training; Learning Essential Approaches to Palliative and End of Life Care (LEAP) is an asset
- 3 years of recent, relevant experience required
- Experience working with and an extensive knowledge of Indigenous people?s language, culture, customs and traditions. Lived experience is preferred
- Indigenous cultural competency training
- Experience or knowledge of the healthcare system and community-based resources, including experience with and/or knowledge of the federal Non-Insure Health Benefits (NIHB) program
- Good interpersonal, oral and written communication skills
- Proven ability to work as a team member
- Work experience in Emergency Services preferred
- Fluency in an Indigenous language would be an asset
- Demonstrated problem solving and critical thinking
- Demonstrated time management and ability to work autonomously
- Excellence in communication, partnership building and inter professional care commitment
- Demonstrated practice and commitment to the principles of patient and family centered care
- Demonstrated practice and commitment to patient and staff safety at LHSC

- Demonstrated ability to attend work on a regular basis
**Immunization Requirements**:
- Provide vaccination records or proof of immunity against measles, mumps, rubella and varicella (chicken pox), Hepatitis B, Tetanus/ Diphtheria/Polio; Meningitis.
- Provide documentation of the Tuberculosis skin testing (two step) and COVID vaccine series.
