Position Summary
- The Social Worker for the North Simcoe Muskoka Regional Children's Complex Care Navigation Program will play a crucial role in supporting families and children with complex care needs. This position is responsible for providing direct clinical services, including crisis intervention, counselling, and facilitating navigation through various healthcare and community services. The Social Worker will work within a multidisciplinary team to ensure comprehensive, holistic care and support for patients and their families.

PRIMARY RESPONSIBILITIES:

- Provide immediate support and stabilization to children and families experiencing acute distress, ensuring timely and effective crisis management.
- Conduct therapeutic interventions both bedside and in outpatient settings to help families navigate the emotional, social, and psychological challenges associated with complex medical conditions.
- Facilitate access to healthcare and community resources, assisting families in understanding and navigating the array of available services.
- Advocate strongly for the needs of children and their families within the healthcare system and external social services, ensuring their rights and needs are prioritized.
- Address ethical issues and child protection concerns, upholding the highest standards of safety and welfare for every child.
- Lead and contribute to the training and development of new staff and learners, creating a supportive community of practice among healthcare professionals.
- Engage in clinic visits, intake meetings, and multi-disciplinary team meetings, promoting an integrated approach to patient care.
- Travel between two sites as required, ensuring consistent and comprehensive care across locations in our region.
- Conduct virtual visits as part of the care strategy, utilizing digital platforms to maintain continuity of care and support for families unable to attend in person.
- Conduct detailed psychosocial assessments to understand family dynamics, strengths, and challenges.
- Develop and implement care plans that address family adaptation, coping, and quality of life in collaboration with healthcare teams.
- Offer expertise in parenting techniques and strategies for families with children who have complex care needs.
- Help families navigate and access financial assistance programs to ensure they have necessary support.
- Build and maintain networks with community agencies to enhance resource availability.
- Keep accurate records and documentation of social care plans.
- Coordinate program activities and contribute to policy and procedure development.
- Educate families, students, and healthcare professionals on social work practices related to complex care.
- Stay updated with new research and evidence-based practices for patient assessment and treatment.
- Provide mentorship to social work students and team members.
- Maintain professional standards, licensure, and competency in social work practices and computer skills.

**Qualifications**:

- Education:

- Current registration with the Ontario College of Social Workers and Social Services Workers required.
- Masters of Social Work required.

**Experience**:

- Minimum of three years of clinical experience with infants and children.
- Demonstrated collaboration skills within interdisciplinary teams and community partners.
- At least three years of work experience in an inpatient or community setting.
- Experience working with children and adults with medical and mental health conditions.
- Ability to solve problems independently.
- Strong interpersonal skills to serve as a lead member of the multidisciplinary team.
