Position Summary
Under indirect supervision, and working as part of an interdisciplinary team, the Social Worker of the Paediatric Diabetes Education Centre is responsible for providing high quality professional counselling to assigned patients, consistent with the philosophy of Orillia Soldiers’ Memorial Hospital.

The Paediatrics Diabetes Centre will work with a targeted population of individuals with diabetes up to and including age eighteen (18).

**PRIMARY RESPONSIBILITIES**:

Patient Care
- Providing comprehensive psychosocial assessment, which identifies child and family strengths, problem-solving abilities, dynamics, interactions, attitudes, feelings and cultural issues in relation to diabetes and resources available to the family.
- Assisting the family to develop a plan of intervention, which addresses coping, adaptation, compliance and quality of life, in collaboration with the team
- Planning, implementing and evaluating the plan of care.
- Utilizing a variety of modalities (individual and group) to provide intervention to assist families, such as clinical counseling, consultation, case/crisis management, adjustment to illness and grief process, community resources and patient advocacy.
- Collaborating with the multidisciplinary team.
- Providing a resource to other staff members and utilizes a research based practice.
- Assessing risk factors for abuse, mental illness and suicide.
- Liaising with community agencies.
- Developing a care plan in collaboration with the paediatric health care team.
- Documenting the social patient care plan. Maintaining accurate records and statistical data.

Program Co-ordination
- Participating in program development including policy and procedures.
- Educating children and families, students, and health care professionals.
- Participating in weekly multidisciplinary patient care rounds and monthly core meetings.
- Perform other duties as may be assigned by the Program Manager.

Professional Development
- Obtaining and maintaining PDEC competency and standards.
- Remain current and aware of opportunities to implement new, evidence-based methods of patient assessment and treatment.
- Attend appropriate workshops, professional conferences, in services and participate in e-learning, Journal and literature reviews as required to update knowledge and skills pertaining to diabetes education management. Update staff not able to attend.
- Participate in clinical projects/studies as required with completion in timelines specified.
- Maintaining proficiency in computer skills.
- Maintain college and governing body license.

**Qualifications**:

- Current registration with the Ontario College of Social Workers and Social Services Workers required.
- Masters of Social Work required.
- Minimum of three (3) years experience working within a hospital paediatric setting is required.
- Knowledge of and experience with utlilizing a variety of therapeutic techniques including Cognitive Behavioural Therapy (CBT), Solution Focused and Strength Based approaches.
