**Job Description**:
***: Under the general supervision of the Community Program Developer is responsible for developing rapport with, and acting as a support for neighbourhood residents. Working with the Healthy Kids HOME Program Coordinator (Thunder Bay District Health Unit) to develop, coordinate, and oversee programming; and liaise with community organizations, project partners, fellow Neighbourhood Leads, and the Healthy Kids Steering Committee is also a requirement for this position.

**MAJOR RESPONSIBILITIES**:

- Collaborates with Division program areas to facilitate neighborhood-based recreation programs that will engage priority neighbourhood families.
- Collaborates with steering committee organizations to schedule programming that neighbourhood planning team identifies as needed within priority areas of nutrition, physical activity, and tobacco control or other health promotion areas.
- Connects families with needed community services and programs.
- Develops and maintains relationships with other service providers in the community; liaises and consults with service providers and neighbourhood families to determine specific needs of priority neighbourhood families.
- Works closely with staff from other priority neighborhood sites to develop and share resources.
- Recruits and engages participants to programs and Healthy Kids sites by building trusting relationships and learning about the families who live in the priority neighbourhoods, including through outreach activities such as door-knocking.
- Coordinates food programs such as community meals, community kitchens and access to emergency food in collaboration with relevant service providers.
- Prepares and monitors Healthy Kids sites, including securing necessary supplies and equipment, identifying needs for alterations, maintenance or repair, conducting safety inspections; ensuring that rules and regulations are followed with respect to staff, service providers, and participants.
- Develops procedures and operational manuals as needed.
- Assists with data collection, gathers feedback regarding community needs and assets, and maintains documentation required for program evaluation.
- Participates in regular meetings with City of Thunder Bay Community Program Developer, Healthy Kids HOME Program Coordinator, other Neighbourhood Leads and Planning Team, and attends other relevant meetings when requested.
- Performs such other related duties as may be assigned.

**EDUCATION/EXPERIENCE**
- Diploma in a relevant human services field such as recreation, health promotion, social work, community development, counseling, community mental health, child development/family relations or related field, together with one to three years’ directly related experience
- Knowledge of community resources (e.g. social services, recreational providers)
- Knowledge and understanding of flexible, diverse, traditional and non-traditional support, advocacy and intervention processes
- Knowledge of equity, inclusion and resilience
- Knowledge of community safety and well-being challenges

**Skills/Abilities**:

- Ability to communicate effectively and concisely, both orally and in writing
- Strong research, project/time management, analytical and problem-solving skills
- Proven ability to function and contribute as an effective team member and to work independently
- Ability to work in a diverse and dynamic environment, while respecting and valuing the diversity of communities and individuals
- Ability to exercise independent judgement when dealing with confidential or sensitive issues
- Must be able to deal with the public in a courteous and tactful manner
- Must be competent within the meaning of the Occupational Health & Safety Act
