**Family Physician - Primary Care Outreach**
**(full and part time positions available)**
Sanguen Health Centre provides health and social care for people who face barriers to accessing traditional care, including people experiencing homelessness and people who use drugs.
We are expanding our Primary Care programming and seeking Physicians with a passion to ensure universal access to quality, community-based and mobile care from a harm reduction, person-centred approach. This is an opportunity to join an existing team of Clinicians specializing in Psychiatry, Infectious Disease (including Hep C and HIV), Safer Supply and Primary Care. These roles are well supported by multi-disciplinary positions such as Harm Reduction and Social Support Workers and embedded within an alliance that allows for advocacy and planning around the broader determinants of health.
Sanguen has recently been funded for 2 full time (or multiple part-time) salaried Family
Physician positions to offer outreach Primary Care service in the places where those who may be homeless or precariously housed live, sleep and eat within KW. These positions will have the opportunity to play a role in planning and supporting innovation and improvement in delivering care to this underserved population.
**Scope of the Position**:
To provide Primary Care services (both episodic and ongoing care with continuity) to those who may be homeless or precariously housed including: preventative health care; chronic disease care; palliative care; infectious disease care including outbreak support; infirmary based care; mental health care including complex mental illness (in collaboration with Psychiatrist on the team); wound care; multidisciplinary team case planning for those with severe substance use problems and severe behavioural problems as well as the possibility of a preceptor role for students.
**Education and Qualifications**:
- Medical degree from a recognized university
- CFPC Certification or equivalent practical experience
- General license to practice from the College of Physicians and Surgeons of Ontario (CPSO)
- Membership in the Ontario Medical Association (OMA)
- Membership in the Canadian Medical Protective Association (CMPA)
- Demonstrated experience with vulnerable, complex populations
- Must be comfortable being around, discussing and working with people who use or have used drugs and compassionate about the barriers faced by people who use drugs
- Must have knowledge of trauma-informed and harm reduction approaches
- Demonstrated experience working in an interdisciplinary teamwork environment
- Demonstrated experience working effectively with culturally and economically diverse populations
- Demonstrated ability to communicate effectively, both written and verbally
**Payment and Working Conditions**:
These are outreach positions that will provide service within community based locations (for example drop-in centres, shelters, a mobile Primary Care Bus). The team will operate during day time hours with some early evening hours. Occasional on-call support may be provided by this team. These positions are remunerated through OMA determined Sessional Rates and positioned within Sanguen Health Centre. Physicians are funded through an agreement with the Ministry of Health and the Ontario Medical Association and will be governed by a Governance Agreement established and maintained by the members of this group.
