**Overview**
**Responsibilities**
- Assist clients with Activities of Daily Living (ADLs), including bathing, dressing, grooming, and mobility support
- Monitor and observe patient conditions, reporting any changes or concerns promptly
- Administer medications and manage medication records in compliance with HIPAA regulations
- Operate Electronic Health Record (EHR) and EMR systems accurately for documentation purposes
- Support patients with memory care needs such as Alzheimer's and dementia care
- Perform medical procedures such as vital signs measurement, catheter care, and CPR when necessary
- Use assistive devices like Hoyer lifts to ensure safe patient transfers
- Prepare nutritious meals and assist with meal planning tailored to dietary restrictions
- Conduct light housekeeping tasks including cleaning, laundry, and maintaining a safe environment
- Document medical records accurately and maintain confidentiality per HIPAA standards
- Provide emotional support and companionship to enhance patient well-being
**Requirements**:
- Prior experience in home health, hospice, nursing home, or assisted living settings preferred
- Knowledge of medical terminology, medication administration, and caregiving best practices
- Certification in CPR is required; additional certifications such as Alzheimer’s or dementia care are a plus
- Experience working with individuals with developmental disabilities or disabilities is advantageous
- Ability to work independently while following care plans and protocols
- Strong communication skills for effective interaction with patients, families, and healthcare teams
- Ability to operate EMR/EHR systems efficiently for documentation purposes
- Valid driver’s license preferred for travel between client locations
- Compassionate attitude with a focus on patient safety and dignity
