**Responsibilities**
- Assist clients with Activities of Daily Living (ADLs), including bathing, dressing, grooming, toileting, and mobility support
- Monitor patients’ vital signs and observe changes in health status to report promptly to healthcare professionals
- Administer medications accurately following prescribed care plans and HIPAA privacy regulations
- Prepare nutritious meals tailored to dietary restrictions and preferences, ensuring proper food safety standards
- Operate mobility aids such as Hoyer lifts or wheelchairs safely during transfers and repositioning
- Maintain detailed documentation of care provided, patient observations, and any incidents in compliance with healthcare regulations
- Support clients with cognitive challenges by implementing memory care strategies for conditions like Alzheimer’s disease and dementia
- Perform light housekeeping tasks including cleaning living areas to promote a safe and hygienic environment
- Collaborate with healthcare teams to develop and follow personalized care plans aligned with each client’s needs
- Engage in patient observation to identify potential health concerns or behavioral changes requiring intervention
- Provide emotional support and companionship to foster a positive environment for clients
- Assist with community integration activities or outings as appropriate for the client’s mobility and health status

**Qualifications**
- Proven experience in home health care, assisted living, or long-term care environments; group home experience is a plus
- Knowledge of patient monitoring techniques, vital signs measurement, and basic nursing procedures
- Certification in CPR and First Aid is required; additional certifications such as HIPAA training are preferred
- Familiarity with dementia care, Alzheimer’s care, hospice support, autism spectrum disorder support, and working with individuals with developmental disabilities or disabilities in general
- Ability to lift heavy objects safely using equipment like Hoyer lifts when necessary
- Strong communication skills to effectively coordinate with healthcare providers, families, and clients
- Compassionate attitude with a passion for caregiving and improving clients’ quality of life
- Flexibility to work various shifts including evenings, weekends, or overnight hours as needed
