Under the leadership of our CEO, Rob Gillis, our organization exists to ensure that residents engage fully in life at Haro Park Centre. Haro Park Centre includes independent and assisted living suites as well as 154 complex care suites for those who need 24/7 assurance and support. Reporting to the Director of Care, Christy Kearney, the Social Worker is responsible for providing professional social work facility wide including: - pre-move-in data collection - move-in support to applicant and families - continuous support and assistance throughout the move-in and “settling-in" period - ongoing supportive services to residents and families - counselling of individuals and/or groups when **Scope** The Social Worker is an integral member of our interdisciplinary team, and provides input/support to problem-solving and decision-making concerning resident issues/concerns. Services are delivered to residents at Haro Park Centre from diverse socio-economic, cultural & ethnic backgrounds. Haro Park Centre is an LGBTQ friendly community. **Responsibilities** - In conjunction with the Interdisciplinary Team, the Social Worker plans, organizes, implements and evaluates Social Work services facility-wide. Assists in establishing service objectives, policies, procedures, standards and - Responsible for pre-move-in process: - interviews residents/applicants - provides information, tours and support to resident and family - ensures all necessary data-bases are completed and documented on the resident healthcare record for new move in. - liaises with families, physicians, and community/hospital workers as necessary. - Provides adjustment counselling to residents and Assist residents and families to understand procedures and services outlined in the Resident Handbook. - Provides ongoing counselling services to residents and families for any social, environmental, economic and emotional - Facilitates resident and family educational/ support - Identifies areas of need and opportunities for growth/support in residents, families and - Provides assistance to residents regarding macro-level issues, including correspondence, completion of forms, communication with government agencies, Assists residents to utilize community resources. - Documents in residents' health records, as per established format and facility - In conjunction with the Executive Director, Interdisciplinary Care Services, develops, implements and evaluates quality management program for Social Services, in accordance with facility-wide - Prepares and maintains all required statistics, records and reports. - Ensures safety of residents during all tasks and reports any adverse events or near-misses immediately. - Maintains level of professional knowledge/skills through review of current literature, contact with peer professionals and attendance at conferences/seminars. - A thorough understanding of community resources such as Public Guardian and Trustee of BC (PGT), The Bloom Group, CPP, Old Age Security Supplements, and applicable provincial and federal ministries that support older adults. - Performs other related duties as **Who Are You?** While fulfilling the many other day to day responsibilities, you: - are sensitive, passionate and it is your mission to serve and support older adults. - value our residents’ dignity and treat them with respect - promote and maintain a harmonious home environment. - encourage family and friends’ involvement in the lives of our residents. - are able to role model conflict resolution strategies. - have a unique and supportive ability to interact with residents, families and members of the community. - possess the necessary skills to interact with internal and external contacts with ease. - work collaboratively within a creative interdisciplinary team. - get along with others and are committed to providing a high quality of service. - believe in life-long learning and maintaining your professional expertise. - support individual personal identities within a diverse community. **Your Education and Experience Should Look like This** - A Bachelor’s degree and current registration with the BC College of Social Work. - A minimum of 2 years’ experience working with older adults in a health care environment or in case management working with geriatric clients and adults with chronic illness. - Experience in providing Social Work Services to socio-culture diverse populations. - Experience establishing, developing and managing productive and cooperative partnerships with community-based and acute care services. **Some additional knowledge that we want you to have**: - Knowledge of Social Work Code of Ethics and Standards of Social Work practice
