**Job Description**:
Reporting to the Recreation Manager, the QLA will:
- Plan, implement, evaluate, develop, and revise both individual and group activation programs based on assessed needs under the guidance of the manager.
- Set up and clean activity area as necessary and transport residents to and from programs.
- Monitor the behavior and condition of residents during programs and report any significant changes to the manager and nursing staff as appropriate.
- Maintain program and resident records according to department policy. Report, document and communicate participation by residents to registered staff.
- Work collaboratively within the interdisciplinary team.
