**Job Title: Family Preservation Worker**

**Hours: 35 hours per week, flexible field hours**

**Location: Ministry of Social Services Yorkton Boundary Area**

**Start Date: July 7, 2025**

**The Family Preservation Worker, under the direction of the Family Preservation Program Manager, focuses on providing supports and services to vulnerable and at-risk families, through flexible and intensive home visitation, referrals and collaboration with other community services, to assist families to develop the skills necessary to keep children safe and to keep the family intact when it is possible to do so. This position will collaborate with members of an interdisciplinary team to plan, implement, coordinate, and evaluate client care and support in consultation with individuals and their families.**

- Provide a high standard of care and support for clients and establish a trusting, mutually respectful relationship with families;
- Schedule, prepare for and attend regular in-home visits with families that meets the family’s needs;
- Provide relevant resources, education, opportunities, motivation and support to families, using a strength-based approach;
- Assist with and provide transportation if needed;
- Refer to and coordinate with other agencies as required;
- Complete reporting standards in a quick, efficient and accurate manner;
- Adhere to confidentiality and safekeeping of client’s files and records;
- Observe program and organization policy and procedure and OH&S standards.

**Qualifications, Experiences and Requirements**:

- A post-secondary education in a human service related field or an equivalent combination of work experience and education
- A Valid Driver’s License and a safe, reliable vehicle
- A current and clean Criminal Records/Vulnerable Sector Check
- A minimum of two-years experience working with children and families;
- Experience in case management, documentation and file maintenance;
- A recent (6 months or less), clear Criminal Record Check and Vulnerable Sector Check.
