Health Access Thorncliffe Park (HATP) is an innovative partnership between Flemingdon Health Centre (FHC) and The Neighbourhood Organization (TNO), and we are seeking a **Peer Support Worker** to join our team in **full-time Contract** role. TNO is a not-for-profit community based multicultural and multi-service agency, located in Toronto, Ontario, with a vision of neighbourhoods where everyone belongs and succeeds.

The Peer Support Worker position requires providing one-on-one services to clients, offering referrals to community and external resources, and delivering accurate information based on client needs. The position encompasses conducting comprehensive client needs assessments and collaboratively developing service plans. Responsibilities also include intake and assessment of newcomers, ensuring client safety, addressing feedback, maintaining databases, and submitting reports as needed.

**Main Duties & Responsibilities**
- Engage clients, assess their needs and provide social, emotional, informational support in their integration and wellbeing.
- Provide support to clients requesting information resources related to mental health, health, economic, social, cultural, educational, recreational, and other services.
- Providing interpretation to the clinical team in the community spoken languages, and share information about services and programs.
- With language and cultural skills be able to identify, remove obstacles and barriers, supporting connections and building community.
- Maintain records, submit reports, and stay updated on immigration and health policies.
- Maintain contact with teams internally and externally, who provide social services, community engagement, health promotion and clinical care.
- Promote the services and programs, especially the access clinic, in the community as well as support, and participate in outreach activities.

**Screening** - As a condition of employment, the successful applicant must complete a Vulnerable Sector Screening.

**Job Type**: Full time contract until March 31, 2025.

**Schedule**: It is expected that the incumbent will work a regular schedule for 35 hours per week (7 hours per day, excluding the lunch hour), with flexibility between 8 am and 8 pm on weekdays, and weekends as required by the HATP/TNO programs/departments.
