**Position Summary**
The **Disability Community Support Worker** provides **individualized, hands-on support** to adults with disabilities in **home and community settings**. This role focuses on promoting **independence, safety, well-being, and active participation** using **person-centred care practices**.
**Key Responsibilities**
- Provide **personal care assistance**, including **hygiene, dressing, grooming, and meal preparation**:
- **Administer or prompt medications** according to established care plans
- Offer **companionship, emotional support, and positive encouragement**:
- Assist with **mobility, transfers, and lifting** to ensure safety and comfort
- Support **community participation**, including **social outings and recreational activities**:
- **Observe, document, and report** changes in health, behaviour, or support needs
- Maintain a **clean, safe, and organized living environment**, including **light housekeeping**:
- Complete **accurate documentation** related to daily supports, progress, and incidents
- **Collaborate with team members, families, and supervisors** to deliver high-quality care
**Qualifications & Requirements**
- **Experience in disability support, community care, or home support** (preferred)
- Knowledge of **person-centred and respectful support practices**:
- Ability to assist with **personal care, mobility, and lifting**:
- **Strong communication, teamwork, and interpersonal skills**:
- A **compassionate, patient, and reliable** approach to care
- **Clear Vulnerable Sector Police Check** (completed within the last 6 months)
- **Valid First Aid & CPR Certification** (Basic or Standard)
- **Legal entitlement to work in Canada** (valid work or study permit, if applicable)
- **Flexible availability**, including **evenings and weekends**
