**Overview**

**Duties**
- Assist clients with Activities of Daily Living (ADLs) such as bathing, dressing, grooming, and toileting.
- Provide patient care and support in a respectful and dignified manner.
- Monitor and document vital signs and changes in clients' conditions.
- Prepare meals and assist with meal planning according to dietary needs.
- Maintain a clean and safe environment by performing light housekeeping tasks.
- Administer medications as per care plans and ensure compliance with HIPAA regulations.
- Engage clients in social activities to promote mental and emotional well-being.
- Use equipment such as Hoyer lifts for transferring clients safely when necessary.
- Collaborate with healthcare professionals to develop and implement individualized care plans.
- Provide support for individuals with Alzheimer’s, dementia, autism, or other disabilities.

**Skills**
- First aid and CPR certification are preferred.
- Experience in home health, hospice care, or long-term care settings is beneficial.
- Knowledge of patient observation techniques and monitoring practices.
- Ability to perform heavy lifting when assisting clients with mobility needs.
- Familiarity with memory care strategies and techniques for working with individuals with developmental disabilities.
- Strong communication skills to effectively interact with clients, families, and team members.
- Compassionate approach to caregiving that prioritizes the dignity and respect of all individuals served.
- Experience in cooking and meal preparation tailored to individual dietary requirements is a plus.
- Understanding of social work principles as they relate to patient care. This role requires dedication, empathy, and a commitment to enhancing the lives of those we serve through quality support services.
