Lower Limb Preservation Clinical Consultant

The Downtown East Toronto Ontario Health Team (DET OHT) is a group of health and social service partners who have deep roots in the community and is committed to creating an integrated system of care. The DET OHT has been selected and funded by Ontario Health to implement a demonstration Integration Care Pathway project for at a Lower Limb Prevention Demonstration Project with the goals of reducing diabetes and/or peripheral artery disease (PAD) related foot complications through increased identification and timely care intervention when foot complications do occur.

To support this work, Unity Health Toronto, on behalf of the DET OHT, is hiring a Lower Limb Prevention Clinical Consultant to support the development of deliverables for Ontario Health. This includes, but is not restricted to
- completing an environmental scan to asset inventory of lower limb preservation services currently offered in Downtown East Toronto
- identifying leading practices in lower limb preservation most relevant for Downtown East Toronto residents
- Completing a SWOT analysis to assess the strengths, weaknesses, opportunities and threats of a novel limb preservation strategy in the Downtown East Toronto

**This is a 0.5 FTE role, for an expected 2-3 months’ time frame with the possibility for extension.**

**QUALIFICATIONS**
- A minimum of five (5) -ten (10) years of clinical experience in Lower Limb Preservation in the Toronto area
- A Master’s degree in Nursing, Chiropody, Occupational Therapy, Physiotherapy or another health professional discipline
- Expert level knowledge of and experience navigating Lower Limb Preservation services
- Direct experience coordinating inpatient and outpatient care services for people with diabetes and/or PAD.
- Demonstrated ability to work in an unstructured ambiguous care environment
- Demonstrated experience in facilitation, and collaboration with multiple stakeholders/service providers required;
- Excellent verbal and written communication skills with the ability to communicate information/ideas, in a manner that is easily understood by others required;
- Excellent interpersonal and listening skills with the ability to effectively interact with staff all levels required;
- Excellent computer skills e.g., Microsoft Word, Excel and Powerpoint, other please specify, required;
- Excellent presentation and negotiation skills with the ability to influence others required;
- Ability to work efficiently and effectively as a team player required;
- Ability to take initiative and work in a self-directed manner as required;
- Ability to interact with people from all disciplines/levels, be flexible/adaptable to change and maintain confidentiality required;
- Ability to work well independently and collaboratively as a team player with staff/stakeholders to collect, examine, consolidate and present information in a clear manner, ensure key factors are highlighted
