EDUCATION
- Valid registration with the College of Social Workers and Social Service Workers is required.
- Master’s Degree in Social Work required
- Completion of the Non Violent Crisis Intervention (NVCI) Program

EXPERIENCE
- Minimum 5 years recent related experience working with older adults with mental health and addictions issues
- Demonstrated clinical leadership skills with a proven track record of achieving results and sustained outcomes
- Advanced knowledge and skills in completing comprehensive psychogeriatric screening assessments and consultations
- Excellent understanding of the geriatric population, including Alzheimer’s Disease and related dementias in a variety of care settings
- Superior knowledge of the long term care sector, community services, resources and program with the sub-region where employed
- Superior interpersonal, coaching and communication skills to effectively work with front line clinicians, patients, families long term care home staff and community partners
- Demonstrated experience with program development, evaluation and research

SKILLS:

- Advanced ability to promote the values of shared leadership and interdisciplinary team collaboration
- Ability to demonstrate a facilitative teaching approach with patients, families, staff and community partners, and a good working knowledge of adult learning principles
- Excellent problem solving skills, critical thinking and communication skills ( both verbal and written
- Computer literacy with proficiency in Microsoft Office Suite
- Valid Ontario Class G driver’s license and reliable vehicle for travel.
