**Position Description**:
We are recruiting for a Transition Navigator to join the Transition to Adult Care Program
The Transition to Adult Care Program is a new initiative that seeks to support youth with complex, chronic illness, who often experience challenges with transitioning from pediatric to adult care. As SickKids moves towards a more individualized model of care powered by Precision Child Health, we have a duty to design and implement solutions that bridge unprecedented outcomes from childhood to adulthood. The Transition Navigator will work within an interdisciplinary team and play an instrumental role in coordinating care, empowering youth and enabling providers across the continuum of care to assume collaborative care management of youth in the community and primary care settings.
This is an exciting opportunity to forge a new role within the health care team and innovate to find optimal solutions. We are looking for someone who is extremely motivated, flexible, and able to act as a change agent to fulfill this role.
**Here¿s What You¿ll Get to Do**:
- Assist in the development of a new model of care that supports youth with chronic illness, rare disease and social complexity transition to adult services.
- Participate in intensive care coordination for time limited period to ensure adequate care through transition.
- Integrate research into practice.
- Co-develop care plans with youth to reflect transition goals and enable connection to adult primary care providers (PCPs) and specialists.
- Assess youth transfer needs (e.g: readiness, preparedness, adherence).
- Empower youth through building self-management skills and motivational interviewing.
- Ongoing partnership and collaboration with primary care practitioners, adult care specialists to ensure a seamless transition.
- Support referrals and joint appointments between pediatric and adult subspecialists (virtually or in person, as appropriate)
- Troubleshoot transition related issues.
**Here¿s What You¿ll Need**
**Essential Requirements**
- You have a Masters degree in Nursing or you are an Occupational Therapist
- Registration with the appropriate professional college (ie: College of Nurses Ontario or College of Occupational Therapists of Ontario)
- Experienced in the area of adolescent health (including chronic conditions and high risk behaviors), as well as the area of health care transitions.
- Working knowledge/experience in Primary and/or Adult care systems
- Ability to navigate heath care resources and manage partnerships in both hospital and community settings.
- Demonstrated initiative and ability to be self-directed as well as work as part of an interdisciplinary team.
- Ability to work effectively with youth who are undergoing stressful life experiences with a trauma-informed lens.
- Excellent interpersonal, communication, organizational, time management skills and attention to detail
- Demonstrated ability to use critical thinking and creative problem-solving skills.
- Strong advocacy skills and motivation to pursue systems level change.
- Commitment to practice in a manner that is consistent with the philosophy of continuous quality improvement and evidence-based practice.
- Understanding and commitment to EDI informed practices
