**Schedule: Monday - Friday, 7:00am - 3:00pm**

The Elizabeth Fry Society of Greater Vancouver (EFry) is a charitable organization that supports women, girls, and children at risk, involved in or affected by the justice system. Our programs work to break the cycle of poverty, addiction, mental illness, homelessness, and crime.

**We are recruiting for a Full Time, Patient Care Coordinator for our Firth Program. ** Located in Abbotsford, the Firth Program serves women with severe and high-risk substance use dependency as a primary concern and who may have co-occurring moderate mental health care needs. The average length of stay will be 3-months, or longer when clinically indicated. The program is strengths-based, trauma-informed, culturally safe, and concurrent disorder capable care delivered through evidence
- based therapeutic interventions. The goal of the program is to provide evidence-based psychosocial treatment, education, and support to reduce problematic substance use, improve overall health and social wellbeing, and ultimately reduce the risk of death due to overdoses and infections. The program will feature both stabilization and therapeutic/treatment interventions.

**KEY RESPONSIBILITIES**:

- Coordinate client care related activities and provide leadership in the planning and implementation of innovative and creative approaches for the delivery of services.
- Assist Program Leadership team in operationalizing care standards, client care guidelines, protocols, clinical pathways and organizational policies.
- Attend and participate in initial client care evaluations and admission meetings as a member of an interdisciplinary team by providing input on a variety of care issues, practice effectiveness and administrative decisions according to client care standards.
- Review client progress and client/family needs. Provide formal and informal consultation as needed to health care team members, other health care providers and students. Provide guidance/knowledge in the management of care in a client/group in defined area of clinical expertise. Promote interdisciplinary collaboration for treatment concerns, continuity of care, crisis management protocols and community resource coordination. Assist with therapeutic interventions and problem solving, provides clinical guidance and information on complex cases, discusses client care issues and develops linkages/partnerships to meet client need.
- Establish, maintain and enhance therapeutic relationships based on respect with client. Engage others in treatment planning and evaluation, provides follow-up planning, support patients/clients to manage self-care, encourages informed decision making and empowers client to improve quality of life. Teach client about management/treatment of medical conditions.
- Monitor the day-to-day operations of residence to ensure quality control in adherence to licensing regulation.

**QUALIFICATIONS**:

- Current practicing registration as a Registered Nurse or Registered Psychiatric Nurse with the British Columbia College or Nurses and Midwives (BCCNM).
- Completion of post basic specialty nursing certificate/program if applicable, and three (3) years' recent, related experience in the designated clinical area, including working with the applicable population group when indicated, and one (1) year's related administrative experience or an equivalent combination of education, training and experience.
- Understanding of mental disorders, addiction and concurrent disorders and treatment, and of the principles of recovery.
- Strong knowledge of Psychosocial Rehabilitation methodology, chemical dependency and addictions treatment, psychopharmacology (indications and side-effects) and psychotherapeutic and counseling skills.
- Demonstrated ability to provide effective leadership, supervision, work direction and consultation.
- Knowledge of culturally safe care, and gender-informed service delivery.
- Experience working in a licensed facility is an asset.
- Excellent communication skills both written and verbal.
- Valid driver’s license (Class 5) with satisfactory driving record an asset.
- Ability to lift up to 30 lbs.

**ADDITIONAL JOB REQUIREMENTS**:

- Standard First Aid with CPR-C.
- Evidence of COVID-19 vaccination status.
- Evidence of a Tuberculosis Test and compliance with the TB Control Program.
- Doctor’s Note of fitness required.
- Enhanced Security Screening & satisfactory Criminal Record Review (Vulnerable Sector Search).
