**GENERAL DESCRIPTION**:The Case Management Coordinator, a member of the leadership team within the Urban

Health and Homelessness Services department, is responsible for direct supervision of 7-8 FTE positions, daily support for the voluntary trustee team, providing back up coordination support to the Corner Drop-in, assisting management in leading and organizing team work and activities, monitoring and coordinating data entry and collection, assisting with completion of statistical reports, and supporting management in a range of other leadership functions. The position engages with partners from a range of community and clinical based settings to address clinical case management issues and foster increased coordination of care.

**MAJOR DUTIES**:

- Provide leadership and direct supervision for assigned front line staff, which includes performance management functions
- Support and assist management in a range of leadership functions related to staff training, program evaluation and development, data collection and entry, and in networking with partner organizations in the community, hospitals and broader social service sectors.
- Assist with coordination of on-going professional development for program staff in areas of complex substance use, harm reduction, mental health, crisis intervention, concurrent disorders and other relevant topics
- Monitor data entry in various database and complete reporting required by funders and supervisor, adhering to PHIPA and TNG-SSCH policies.
- Oversee daily operations of the voluntary trustee program
- Coordinate supervision of students and program volunteers
- Support the Corner Drop-in Coordinator in the running of the program including participating in floor coverage, planning and facilitation of groups and supervising staff, providing back up coordination support
- Intervening and deescalating crisis situations
- Engage in the active team case management and facilitation of clinical consultation meetings
- Directly assist Manager with provision of the program as requested/directed
**REQUIREMENTS & QUALIFICATIONS**:

- B.S.W./B.A. plus 2 years of supervisory experience in human services, including goal setting, workplans and performance appraisals, or combined equivalent of education and experience
- Minimum 4 years’ experience working with marginalized populations, including individuals experiencing homelessness, substance use and mental health issues, with at least one year in a case management position
- Demonstrated leadership skills and experience working on a multidisciplinary team
- Experience, alignment and commitment to harm reduction
- Excellent communication, initiative, judgment, and problem solving skills
- Experience building and maintaining cross-sector collaborative relationships
- Strong understanding, analysis and commitment to anti-racist and anti-oppressive practices
- Ability to work in a dynamic community-based setting with marginalized populations
- Experience in program development, implementation and evaluation is an asset
- Physically able to travel consistently throughout the city on transit
- Proficient with MS Office, and web based data entry for case management and time/labour.
- Demonstrated abilities to multi-task, organizational and time management skills
- Excellent written and verbal skills in English.
- Recent demonstrated knowledge and skills in assessment, crisis intervention and prevention, conflict resolution and negotiation skills, including CPI certification or willing to be trained
- Able to work evenings, weekends and on-call as assigned
