Dual Role Passport Worker Direct Support Professional in Passport

The direct support professional (DSP), under the direction of the team supervisor and manager, will primarily be responsible for working with people they support to help them develop the skills and connections they need to be successful and achieve the level of independence they have chosen.

**Job Summary**:
The employee in this position will provide support to individuals within our Passport Program and within our Supported Living Program.

The employee in this position will provide individualized support in a manner consistent with Mainstream’s philosophies and values. When finding the best supports for people this includes:

- Fostering independence by building on individuals' abilities and developing community participation, and social skills
- Identifying and planning strategies to assist clients in reaching their goals, offering guidance and resources
- Increasing opportunities for participation in the community with supports that respect personal choices and decision-making
- Promoting social inclusion and strengthening social relationships by supporting the use of community resources and services
- Supporting families and caregivers of an adult with a developmental disability to assist them with maintaining their caregiving role
- Collaborating with team members to effectively support people in the Passport and the Supported Living Program

**Job Duties and Responsibilities**:
**Passport**
- Providing education about the Passport Program; linking to services and supports. Working positively and proactively with clients and their families
- Communicating with supported individuals and their families to provide support in using Passport funds to meet the individual’s goals
- Providing information, support and resources to enable the individual to actively participate in decision making and self-advocacy
- Providing transportation for clients, using either your personal or agency vehicle, and accompanying them to appointments, activities, shopping, and other essential destinations
- Facilitating participation in community, recreational, social, and employment activities
- Developing trusting and positive relationships with the person supported and their families
- Maintaining records of activities, progress, and expenses from outings
- Advocating for inclusion, accessibility, and self-advocacy

**Direct Support Professional**
- Supporting activities of daily living such as bathing, dressing, grooming, toileting, and eating
- Assisting with medication management and administration
- Supporting individuals in developing life skills by guiding them in cooking, budgeting, employment preparation, and socialization
- Completing housekeeping tasks such as laundry, meal preparation, grocery shopping, and cleaning
- Maintaining accurate and detailed notes on daily activities, incidents, and changes in the individual’s health

**Qualifications & Key Requirements**:

- College Diploma (Social Service Worker or Educational Assistant - Special Needs support Diploma or related diploma) OR university degree (Bachelor of Applied Science in Community Social Services or related degree)
- Must have a valid Driver’s License (G) and a reliable vehicle; Auto Insurance (with a $1,000,000 liability coverage and a permission to carry non-paying passengers)
- Must have a police clearance (with vulnerable sector screening)
- Must have a valid First Aid, CPR
- Bi-lingual in both French and English is an asset
- **Be available to work one weekend per pay period if required**:

- **Be able to lift up to 30 lbs.**:

- Training in Person Centred Thinking is an asset
- Training in Non-Violent Crisis Intervention is an asset
- Minimum one year working with individuals with developmental disabilities or minimum one year working in a related field

Please note that the selection process will be person centered and utilize matching tools to ensure that employees are well matched to the people they will be supporting, the program, the team and the organization. The selection process will also require you to demonstrate your skills in providing person centered supports and connecting to the community through written exercises, interview questions and hands on activities.

Pay: $26.18-$26.90 per hour
