**Community Support Worker (CSW) Responsibilities for our Transition to Independent Living Program (TIL)**
**Independent Living - Community Support**
- Support with daily living needs (i.e. groceries, medication).
- Securing bank accounts.
- Funds disbursement, including financial mentoring and modules.
- Transportation to appointments/ meetings.
- Support to connect youth with a family doctor, optometrist, and dentist available to them as adults, make appointments as required.
- Ongoing awareness and promotion of various community-based organizations/ government services.
- Facilitate/promote access to community-based programs including, cultural, recreation, training based on interest.
- Refer and connect to Futures Forward, YES, Community Financial Services, Rent Smart.
**Housing Supports**
Housing (securing housing, regular check-ins and tenancy management, landlord relations)
- Apartment searching.
- Regular check-ins for youth already housed.
- Teaching and mentoring home safety and tenancy management.
- Fostering good relationships with property managers and landlords.
- Support in apartment cleaning and restoration.
**Life Skills Programming**
- Teaching and mentoring life skills programming, utilize curriculum and hands on lessons and/or integrated into daily living.
- Assistance and teaching skills such as paying rent, setting up utilities, budgeting, meal planning, grocery shopping and meal preparation.
- Guidance on hygiene.
- Guidance on healthy relationships, personal safety and wellness.
- Support to address any barriers or challenges youth is experiencing.
**Documentation and information sharing**
- Maintain written logs, document pertinent information as it relates to youth activity and programs provided; communicate with team and other supports, adheres to duty to report on significant incidents and/or disclosures.
- Attends and actively participates in case conference meetings.
**Part time, weekends (must be avail Sat and Sun 8-4 or 9-5) and opportunity to pick up casual shifts during the week.**

Pay: $19.67-$20.62 per hour
