An exciting opportunity exists for the right individual to work in our Burnaby Head Office as a Hybrid Supported Home Share Case Manager.

The Hybrid Supported Home Share Program is a wrap-around model of care that Strive provides to individuals with complex behavioural, social, and medical needs. Individuals are placed into carefully matched family care homes and receive a minimum of 30+ hours of one-to-one support per week, as well as ongoing case management. This program offers a structured home environment for individuals who require a high level of specialized support. By having a stable home provided by a contracted caregiver, coupled with extensive one-to-one staffing, an individual can be enveloped in customized service and supported to reach his or her full potential.

**Job Type**:Full-time, Permanent

**Start Date**:ASAP

**Compensation**:starting at $31.53

**The regular hours of work for this position are part-time, forty (40) hours per week, Saturday, Sunday, Monday, Tuesday, Wednesday, times may vary.**

**Job Duties**:
**Key Duties and Responsibilities**:

- Recruitment and selection of frontline staffing in accordance with equal opportunity employment standards, the Human Rights Act, Multiculturalism Act, Community Care and Assisted Living Act, Criminal Records Review Act, and Child, Family and Community Service Act.
- Travel into the community to conduct supervision of the contractors' home and provide support to 1:1 staff while on shift.
- Be available to work 1:1 with supported individuals in the home setting and community.
- Experience working with individuals that live with behaviourial challenges
- Experience/willing to provide mínimal personal care for supported individuals
- Review expectations and contractual obligations with contractors on an annual basis.
- Collaborate with the Program Manager and colleagues.
- Reporting to SIL/Hybrid Program Manager and Director of Operations
- Familiarity with the community living sector
- Post hiring ads on agency specific sites
- Provide program manager with regular updates on Hybrid files, and staffing needs
- Professional boundaries
- Work well within a multidisciplinary team
- Access to computer and internet
- Access to a vehicle

**Job Requirements Include**:

- Criminal record check
- First Aid and CPR
- Minimum of three (3) contactable references
- Valid BCDL and Reliable vehicle to use for work purposes

**Additional Requirements**:

- Experience in the public or non-profit sector
- Experience working with individuals with physical, developmental disabilities, brain injuries, and/or complex health issues
- Experience working in a team environment
- Experience working with a case management team
- Demonstrated ability to deal with crisis and emergency situations
- Computer literacy and experience with databases
- Excellent written and verbal communication skills
- Excellent time management skills
- Knowledge of community resources
- Must be willing to travel throughout Lower Mainland
