**Position Summary**:
The Clinical Practice Lead supports Covenant Care and Covenant Living’s (CC/CL) strategic goals by focusing on evidence-based clinical practices, providing expert guidance for complex clinical challenges, and collaborating on quality improvement initiatives. This role ensures the alignment of clinical policies and procedures with current standards, promotes resident safety, and fosters a culture of continuous improvement in care quality.
The incumbent is expected to live Covenant Care and Covenant Living’s mission, vision, and values in their day-to-day work, exemplifying a commitment to compassion, collaboration, and service excellence. Professionalism, respect and the ability to cultivate positive and productive relationships with colleagues are essential to the role.
**Primary Role and Responsibility**:Infection Prevention & Control (IPC)**:

- Maintain Covenant Care and Covenant Living’s Infection Prevention and Control program, including but not limited to policies, procedures and processes in accordance with evidence and regulations Creates a collaborative, empowering, and inclusive work environment.
- Develop and deliver training on IPC protocols.
- Conduct regular IPC audits and support communities in addressing identified gaps.
- Monitor infection rates, analyze trends, and collaborate with Quality Improvement leads to address systemic issues.
- Lead and support outbreaks responses in CC/CL communities.
- Represent CC/CL in external IPC committees

**Practice Lead**:

- Model the organization’s mission, vision, and values, consistently representing Covenant professionally and positively.
- Collaborate effectively as a professional team player, supporting the achievement of Covenant’s mission, vision, and values.
- Engage and mobilize teams to fulfill Covenant’s mission, vision, values, and strategic objectives.
- Ensures consistency and excellence in clinical practice throughout CC/CL.
- Drive operational excellence by promoting streamlined processes, operational efficiencies, continuous improvement, enhanced controls and data integrity.
- Implement clinical policies and procedures across all disciplines.
- Oversee the collection, analysis and reporting of data.
- Conduct root cause analysis for clinical incidents and facilitate action planning.
- Provide guidance and support for addressing complex clinical challenges.

Quality Assurance and Compliance
- Maintain CC/CL’s quality assurance program, including but not limited to, conducting internal audits to ensure compliance with regulatory and accreditation standards.
- Work with community leaders to develop and implement action plans to address gaps..
- Monitor the progress of action plans and ensure timely completion.
- Prepare and present detailed reports on external compliance.

General Quality, Practice, and Education (QPE) Responsibilities
- Lead Quality, Practice and Education committees as directed.
- Serve as the Electronic Health Record and Accreditation champion.
- Provide mentorship, leadership, and professional development opportunities to staff.
- Support operational vacancies, such as SA/RCM roles and participate in on-call coverage as required.

**Qualifications**:

- Current registration and good standing as a Registered Nurse or Licensed Practical Nurse with CRNA or CLPNA
- Proven experience in clinical practice, quality improvement or infection prevention in a healthcare or seniors’ care environ
