The Case Manager is responsible for both the casework duties as well as the operational management of the Halifax location. Casework includes processing and administering all aspects of casework including Intake, Move In, Needs Assessment, Case Planning and Move Out. Lastly, the Case Manager is responsible for program management and development including planning and implementing external regional outreach and fundraising activities, internal program management and ensuring cohesive operations on-site with the city team. The outcomes for the Case Manager are twofold: to provide support, security, advocacy, information, education and referrals of clients while developing a relationship that nurtures client independence as well as lead efforts to ensure visibility with donors and community partners.

KEY RESPONSIBLITIES

CASEWORK MANAGEMENT

Intake
- Screen potential residents for intake suitability when required by House Manager
- Participate in the selection and admission of residents to appropriate program
- Review client background information, interview clients to obtain case history and prepare intake reports

Move In
- Provide pick up service for client move-in, if required
- Prepare and assign bed room, cupboard, linens and towels to client(s)
- Provide a welcome pack for the client(s)
- Conduct initial meeting with client to review and sign-off house rules, liability, consent and confidentiality agreements
- Conduct house tour and introduce client to other house guests and staff

Needs Assessment
- Assess client's immediate needs within 2-3 days of client move-in
- Guide client(s) in developing and implementing an action plan for their 3 months stay at the home
- Inform client(s) of various options and resources that are available to meet their needs
- Provide necessary support and assistance for the client to ensure action plan is achieved

Move Out
- Encourage and support clients to secure sustainable housing within the community
- Assist client in accessing various community or governmental programs regarding housing, welfare, childcare, child benefits, disability, English Language training, etc.

CLIENT SUPPORT AND DEVELOPMENT
- Meet with client(s) weekly to assess their progress, give emotional support and encouragement, help with goal setting and problem solving supports
- Facilitate house meetings to review housekeeping issues
- Advocate for and represent the client's needs in house meetings and with other community/governmental agencies
- Encourage clients to abide by house rules through positive reinforcement and role modeling.
- Monitor the clients’ adherence to the house rules and reports all violations immediately to the House Manager.
- Assign and enforce restriction and/or consequences for rule violations.
- Regularly conduct client bed checks.
- Maintain a daily written log regarding house and client activity
- Facilitate resolution of conflicts between client(s) in the home through positive communication and supportive listening
- Organize and conduct workshops to promote mental and physical well-being, life skills and self-care
- Monitor and ensure the safety and comfort of client(s) and the security of the home.

REGIONAL DEVELOPMENT

OUTREACH & EVENTS
- Create and plan community outreach strategy alongside other regional managers
- Organize and facilitate events to engage the local community members and stakeholders
- Maintain current knowledge of issues and resources related to abuse and violence to educate and create awareness about our service offerings
- Facilitate community networking, round tables, and workshops
- Collaborate and communicate with other programs and external partners of the organization, and maintain a database of local resources and services
- Set and meet organizational KPIs for monthly events, collaboration, and meetings with partners
- Facilitate and participate in community networking and committees

COMMUNITY & VOLUNTEER ENGAGEMENT
- Develop and maintain positive relationships with key community partners to initiate advocacy, education, referral, collaboration, and funding opportunities
- Recruit, train, engage and retain volunteers for events, seminars, fundraising or regional activities

FUNDRAISING & DONOR ENGAGEMENT
- Develop and nurture the business relationships with external organizations (NGOs and governmental agencies) including PR and fundraising activities, explore grant giving/seeking and collaboration opportunities, execute referrals
- Arrange for meetings with donors and other funding stakeholders to engage and gain donor support

OPERATIONS MANAGEMENT

PROGRAMMING
- Build and maintain a positive work culture amongst all team members that are reflective of the organization's vision, mission, and company values
