**Intake and Service Needs Manager**(Permanent, Full-time)
**Desired Qualifications**:
- Strong understanding of Indigenous concepts of care and support.
- Bachelor's degree in a relevant field (e.g., social work, social justice, Indigenous studies, health related and/or a combination of pertinent experiences in these area)
- 5+ years of management experience, in a non-profit organization.
- Knowledge of Community Living Disability Services (CLDS) funding model and guidelines
- Solid knowledge of disabilities, developmental disorders, and mental health issues.
- Proven ability to deliver person-centred services addressing physical, emotional, intellectual, and spiritual wellness.
**Responsibilities**:
- Coordinate new referrals and intakes submitted to Oshki-Giizhig. Ensure timely and effective participant service setup. Act as the primary point of contact for new inquiries.
- Meet with participants, families, and collateral contacts to introduce them to the services provided by Oshki-Giizhig, including driving to meet participants, their families, and collaterals in the community.
- Establish a strong relationship with new participants and collaterals, to ensure they are connected to the necessary support.
- Provide guidance to staff in navigating government processes and protocols to secure resources and services for participants.
- Identifying potential areas of service development both internally and throughout the community.
- Collaborate with CLDS and key government and community agencies including social services, health, education, child welfare, private non-profit organizations, justice, and Employment and Income Assistance (EIA) to ensure seamless service delivery and comprehensive support for participant needs.
- Familiarize oneself with CLDS funding guidelines for Oshki-Giizhig's services and tailor proposals to fit within guidelines, based on information obtained during intake assessment and file review.
- Advocate for funding that meets the needs of the participants we support.
- Collaborate with the housing and outreach team to determine availability of services and best fits for service.
- Maintain and track records of participants' funding for placements within the agency.
- Collaborate with the finance team to ensure that invoices match with services provided.
**Additional Requirements**: Criminal Record Check, Adult Abuse Registry Check, Child Abuse Registry Check. Driver’s Licence and access to transportation.
