- Demonstrated knowledge and experience of the healthcare sector, in either a clinical role, or through an industry connected to the health system
- Previous work in a health care environment
- Awareness of PHI and the importance of privacy and security in the health system (preferred)

**ACCOUNTABILITIES (Major Responsibilities of the Position)**
- Participate in special projects of the Chatham-Kent Ontario Health Team which include communications and engagement strategies to implement within the health care system and the general public.
- Work with health care professionals from different organizations to gain buy-in for change.
- Implement activities with participating health service providers, and the general public to smooth the adoption process of an online patient portal.
- Contribute to problem solving resulting in successful awareness, education, adoption and registration of an online health patient portal.
- Utilize strong communication skills working with the public to provide awareness and education of health care projects and initiatives.
- Develop and implement communications and social media strategies for awareness of health care projects and initiatives.
- Conduct presentations on new projects and intiatives with health care professionals within the local health system.
- Conduct implementation activities, document lessons learned and present findings in a logical and easy-to-understand manner.
- Ability to support and embrace the philosophy of Patient and Family Centred Care.
- Synthesize community engagement feedback as input to CKOHT strategy and innovation priorities, ensuring alignment of services with the needs and expectations of community stakeholders.
- Embeds in all OHT related work the shared vision, guiding principles and strategy of the CKOHT, including Health Equity, Diversity and Inclusion.
- Demonstrated participation in engagement strategies and partnering with multiple organizations to improve the patient care processes and systems.
- Monitor, track and complete project reporting requirements.
- Meet targets and deadlines and adapt to project changes and needs.
- Have a positive and supportive service attitude and works well within a team.
- Obtain feedback and liaise between provincial, regional and local partners to meet project objectives.
- Other duties as assigned.
