**Who You'll Work With**

As a Client Care Coordinator, you will be primarily responsible for coordinating onboarding, treatment start & discharge of clients participating in the Mental Health program. You will be part of a highly collaborative team which includes Mental Health Professionals, Customer Success Representatives & Account Managers.

**What You'll Be Doing**
- Work collaboratively with mental health professionals and case managers to smoothly onboard new referrals as well as routinely communicate progress status to case managers via system generated progress reports.
- Verify patient information during intake and coordinate collection of missing information.
- Follow up with clients as and when required.
- Navigate clinician availability & schedule Diagnostic calls with psychologists to ensure timely appointment booking.
- Act as a support for both clients and clinicians - responding to all requests within 24 business hours.
- Complete monthly tracking sheets and checklists.
- Maintain various client trackers, reports and clinician documented treatment summaries.
- Coordinate release of clinical information with clinician and requesting client while also maintaining PHI confidentiality.
- Work cohesively with fellow team members to provide an atmosphere of trust, mutual respect, and cooperation.

**What You Need to Be Successful**
- Minimum of 2 years working in an office setting (healthcare setting preferred)
- Medical Office Admin Certificate/Diploma
- Proficient in Microsoft Office 365 tools

**What You Need to Stand Out**
- Fluent in both English and French is preferred
- Ability to handle a high volume of intakes while delivering exceptional customer service (healthcare industry experience an asset)
- Ability to multitask and prioritize
- Excellent written, verbal, and electronic communication skills
- Critical thinker with strong organizational skills
- Team player & strong interpersonal skills
- Strong data entry skills.
